دكتور اجانه بيشنت agitated و انطيناه Haloperidol بعدها ب فترة قصيرة البيشنت فيف VF و CPR و ادرينالين يلا رجع. شنو ممكن يكون السبب؟؟؟ بالنسبه الي اعتقد هذا الكيس هو Haloperidol-induced Torsade de Pointes which is rapidly degenerated into VF. و المنجمنت تمام التمام و لازم نتذكر نقطتين بهجي كيس 1. Amiodarone shouldnot given, it further prolongs the QT interval. 2. It is mandatory to add Mg sulfate 2 g IV over 2 min و اكيد نوقف الـ Haloperidol. هالوبيردول علاج دائما نستعمله بس ننسى هذا السايد ايفكت، للأسف. #emergency

مريض جايني و عنده malena او مريضه صار عدها post partum bleeding او مريض عنده sepsis بس بعده ما داخل ب septic shock شلون اراقب هذول المرضى بحيث اشخصهم قبل لا يدخلون ب شوك من نجيب الطبلة و نكتب الفايتل ساين SPO2, BP, PR, RR, and body temperature نضيف وياهم عامل سادس و هو Shock index مو تنسى رجاءا Shock index = heart rate/ systolic BP Normally 0.4 - 0.9 اذا لكيت الشوك اندكس اكثر من 1 معناها هذا البيشنت ممكن راح يدخل ب شوووك لازم تبدي تعالجه aggressively قبل لا يروح من اديك. الشوك اندكس و الـ mean arterial pressure MAP جاي انشر عليهم دائما لكن للأسف، بعدهن not utilized بالطوارئ و لا حتى بال ICU. هواي منكم بعدهم يعتمدون فقط ع BP لتشخيص الشوك و هذا جدا جدا غلط. #emergency

Pneumonia: need time هذن يحتاجن وقت، يوم يومين

📑 AV dissociation من المواضيع المهمة .. ومفهومه خطا بان كل AV dissociation هو AV block وهذا خطا لان الها اسبابها وهنه 3 اسباب ويختلفن ب treatment لهذا من تشوف AV dissociation مو مباشرة تكول هي AV block لهذا نجي لل causes : 👉 AV Block (conduction defect) اكثر تججي ويه 3rd heart block وقليل ويه 2nd heart block sever and more distal to infra nodal ————— 👉 Default (when the sinus rate slows to below 80/minute): هنا خلل ب sinus atria بحيث تكون ابطأ من البقية والنتيجة راح يصير escape اسبابها - ischemia - myocarditis - vagal reflexes ————— 👉 Usurpation (due to the slightly accelerated ventricular septal focus): هنا نتيجة accelerated junctional or ventricular rhythm بحيث ما تلحك عليها SA node اسبابها: - Ventricular tachycardia - Drug toxicity (especially digoxin) ————— 👉 Combined هنا تداخل الاسباب وتبقى ع تفسيرك تشوف accelerated OR escape ————— وتنقسم نوعين 👉 Complete »» no communication via AV node 👉 Incomplete »» intermittent atrial capture from the ventricles or »» intermittent ventricular capture from the atria واخيرا AV dissociation is a description, not diagnosis #ECG #cardio
