جواب Contact dermatitis ⏩ double folds below the eyes in pt with eyelid edema as a result of eyelid dermatitis & presence of dennie morgan folds is a diagnostic marker for allergy ( lid eczema with edematous swelling...) with minimal scaling of eyelids هواي رسلوا dx: heliotrope rash n dermatomyositis لهذا راح اوضح heliotrope rash macular, confluent, purple or violaceous erythema on the upper eyelids ( rare cases on lower eyelids) and\or edema عكس ما جاي نشوفه بالبيشنت فد ملاحظة اذا شفنا بس edema بدون erythema & scaling or pruritus نبدا نفكر ب hypothyroidism or angioedema وفد ابروج بسيط لل swollen red eyelid

Skin peeling as a side effect of bisacodyl!!???? ال bidacodyl، الي موجود بالصيدليات ب اسم Dulcolax، احد أشهر ال laxative تقريبا، و يعتبر relatively safe. عندة كلش rare side effect هو hypersensitiviy reaction، مثل ما جاي تشوفون بالصورة. This skin peeling was part of Steven Johnsin syndrome. @AASmedicalbot

This child presented with " I can't put my foot on the floor deeply" for 3 days. No hx of trauma + no hx of surgical or medical issues + no previous hx of GIT or urinary complains (possibility of reactive arthritis). O/E : normal vitals, abdominal, cardiac, chest, and neuro ex. Squeeze test (media-lateral compression on the heel) was positive. Sending for X ray : Sever disease was diagnosed (calcaneal apophysitis) That is it 😑😑

أحد الكيسات الي قل ما نشوفها A 7 yo boy presented with hip pain and limping, the condition started 5 wk ago. و بذاك الوقت مسوين اشعه و كامت نورمل و منطينهم مسكنات، لكن الحالة استمر و slightly progressive. I went so deep in the hx for any preceding trauma, injection, minor illness, polio infection, endocrinopathy, all are - ve. O/E : Vital, neuro, abdominal, cardiac.... All are normal Locomotor ex : marked limitation of left hip movement despite normal neuro ex. Inx : Started with simplest imaging, hip X ray و النتيجة واضحة أمامكم avascular necrosis of femoral head، و الي يسمونها Perthe's disease. و المفروض بالبداية نتأكد بال MRI بعد ما كانت الاشعه طبيعية. بالرغم الاكزامينيشن ما كان اكو short one leg، بس جنت أتصور الأشعه راح تجي slipped capital femoral epiphysis SCFE. Interesting case : A 61 yo F presented to ER with stuporous state (respond to painful stimuli). She is hypertensive but not diabetic. Vital signs : hypertensive, PR nearly 50, and cold, RBS 260, RR 13 بدينا ب الهستري : دكتور هي قبل ٣ ايام بدت تهذي، و يوم عن يوم زادت حالتها و بدت ما تعرفنا، لحد ما هسه فقدت. عدها أمراض مزمنة؟؟ لا، بس ضغط و جاي تأخذ علاجات من الصيدلية!!!! اي سولفلي بعد؟؟، هاي هي دكتور كلشي ماكو بس هذن الي كلتلك بيهن 🙂🙂. O/E : -- general look : obese, buffy face -- neuro : hypo-reflexia in lower limbs + areflexia in upper!!!!!???, normal planter response -- Extremely cold but hypertensive!!!! Neck examination : big goiter 😎 من يمتة هاي، قديمة و ليش ما تكولون، لأن ما بيها ألم دكتور. -- cardio : normal heart sound!!!!??? + no added sound Chest : no Crackles ECG : osborn wave (sign of hypothermi) CASE of Myxedema coma #emergency
