A 6 yo F presented with this skin lesion, apoeared at morning (as the claimed). ال lesion شكيت بيها erythema nodosum (بس هم ما يمشي وياها كلش، شسوي بس هاي جانت ببالي 😂😂) فبديت اخذ هستري مال EN : Any hx of Previous or current respiratory tract infection (S Pneumonia is the most common cause) Any hx of night sweat, wt loss (TB, juvenile Sarcoidosis) Any hx of abnormal bowel habit or Abd trouble (30% of IBD have EN) و استمريت اخذ هستري لحد ما مللت المريض . المهم، البنيه خايفة، ف شغلت والديها بكم شغلة و بقينا انا وياها 100% Privacy. She said : I abused by my dad's wife, my mother is gone in the last 6 mn. So take care in all cases of brusing, always assume child abuse.

كيس مهم و مهم جدا A 23 yo M presented with dizzness and feeling unwell since 15 min, then he got syncope. Vital signs : Radial pulse : low volume, detected paraxysmally!!! BP : 100/60!! RR : 12-14 Cold & sweaty ( So, I sweated also 😂😂) هواي هجي كيسات و نفس الأعراض، يعني مو اميرجنسي كلش (حسب تصوري) ، ف خليني اخذ هستري بين ما هم يسوون ECG : عنده أمراض مزمنة؟؟ لا دكتور كلشي ما عنده!! اكو تأريخ أمراض قلب بالعائلة؟؟ لا،. كلشي ماكو، كلنا صاحيين!!! يأخذ أدوية معينة؟ لا ابدا احد ضربة ع قلبة، وكع ع صدرة (commotio cordis)، لا ابدا، جان عند طبيب أسنان يريد يقلع سنة، و قلعه و رجع تعبان، و حالته جاي تزداد سوءا، قبل لا اشوف التخطيط ما ارتاحيت لأخر فقرة. وين التخطيط أشوفة 🙂🙂🙂 Paroxysmal ventricular standstill due to lidocaine toxicity (most propably) after dental anaesthesia. و آل clinical picture و لا تسوي reflection للمصيبه الموجودة، ما اعرف ليش 😑

Please please remember : What ever the patient's complain (dyspnea, dizzness, vomiting, Abnormal body movement, weakness, feeling unwell, disturbed consciousness, etc), chech blood sugar. Hypoglycaemia has multi-faces in presentation.

A rare, interesting case : 40 yom, presented with pulsatile mass on the snuffbox for the last 8 days. Pmhx : - ve (Completely free from any issue) Fhx : - ve بس ال occupational hx كان significant، لأن المريض heavily hand user و متعرض إلى repetitive hand trauma. طبعا التشخيص واضح هو aneurysm of dorsal brabch of radial artery ال pathophysiology مالها تشبة ال hypothenar hammer syndrome. A rare entity 🙂🙂🙂 هذا منشور عرضي، مو TIA بس تذكرته و نشرته 😌😌
