هذا الطفل عندة constipation من ولادتة، و عمرة ٣٤ يوم.
و مراجعين بيه outpatient clinic، يغيرون الحليب مال الطفل فقط (حسب كلام الأهل).
This infant presented to ER with failure to pass motion for the last 3 days, gases are passed.
O/E : markedly distended abdomen, which is dull on percussion + umbilical hernia!!!!
احتمال هاي umbilical hernia و صار بيها incarceration و سوت intestinal obstruction؟؟
الهرنيا من شوكت عنده؟؟ دكتور صارله اسبوع تقريبا.
و بهاي الفترة ال constipation كان موجود اصلا!!!
يعني هاي الهرنيا هي result of constipation.
الحمد لله، كان ال Suspicion of congenital hypothyroidisn جدا عالي
Which is confirmed later on.
This is a case of hypothyroidisn-induced pseudo obstruction of the intestine.

Thrombocytopenia vs Vasculitis?
🔰Tip to differentiate Thrombocytopenia vs Vasculitis purpura —> Close your eyes and touch the lesions!
🔰Palpable Purpura = vasculitis
Note:
📗 Palpable purpura can also develop as a result of infectious microemboli (as can especially occur in severe Staph. aureus)
📗 Osler nodes and Janeway lesions seen in conjunction with bacterial endocarditis are examples of embolic lesions that can be both purpuric and palpable.

With seconds he started gasping and the level of conscious is lost.
He has hx of IHD, HT, DM, CKD, PCI, stroke, acute limb ischaemia, atrial fibrillation.

هلو شباب/بنات
اخر كم بوست، نشرتهن كتابة بناءاً ع طلب بعض الأصدقاء، لكن راسلوني كم صديق يكولون فويسات أحسن.
يا ريت تصوتون، حتى استمر بعد ع نمط واحد ان شاء الله 😑😑

أحد الكيسات المهمة و ال uncommon تقريبا.
هذا المريض عمرة 22 سنة، عندة hx of idiopathic Epilipsy.
أهلة جابوه بسبب ال status epilipiticus الي استمرت عندة ما يقارب 20 دقيقة و قبل لا يستعيد وعية اجته attack ثانية و هو بالسيارة بالطريق ، بس المهم هو من وصل كان free from attacks، بس عندة post ictal sleeping (أو قد يكون other cause of LOC).
أخذنا هستري، و تبين ان سبب ال status epilipiticus هو بسبب abrupt withdrawal of anti convulsants.
المهم، بعد ما فحصنا الفايتل ساين و النيورو اكزام و سحبنا دم و دزينا لل S electrolytes and renal function test.
بعد ذلك سوينا ECG قبل لا ترجعنا التحاليل اصلا.
Look to V1-2, there is a saddle-shaped ST elevation less than 2mm with Inverted T in V1 and biphasic T in V2 (Brugada pattern type 3)
البيشنت ما عدهم family hx of sudden death و لا قابل صايرة عندة cardiac trouble, فشنو سبب ال Brugada pattern 🙂🙂
This is a pseudo-Brugada syndrome due to hyperkalaemia, which occured due to status epilipiticus ( SE Causes rhabdimyolysis >> release K and myoglobin >> hyper K + acute kidney injury).
و رجع التحليل ال K=8.1
فمو كل Brugada pattern هي Brugada syndrome. فقط إذا كان type 1 pattern فهي تعتبر diagnostic.

تخيل يجيك مريض من الصبح
عمرة ١٧ سنة
و يشتكي من lower limb weakness من البارحة و اليوم
On Examination O/E :
power is 3/5
Normal tone
Normal reflexes??!!!!
No sensory defect
Normal anal tone
صراحة بهذا العمر واحد يفكر بال
1. Gullian Barre syndrome GBS
بس صعبة نكول ع هذا الكيس GBS لأن ال reflexes are spared
2. Hypokalaemic periodic paralysis HPP
سوينة ECG حتى نشوف ال
U wave, flat or Inverted T wave, prolonged QU interval, and tachycardia (signs of hypokalaemia)
اجه النتيجة normal ecg
3. Transverse myelitis TM
بس هذا لازم يكون Flaccid paralysis مو مجرد weakness عادي، و لازم يكون وياه sensory symptoms.
4. Acute peripheral polyneuropathy APPN
اخذنة هستري مال Drug و مال ال organophosphorus exposure و lead exposure، و كلهن - ve.
صراحة واحد يبقى محتار، فقط weakness و الباقي كلشي طبيعي.
فدزينة routine investigations الممثلة بال :
Blood sugar
Renal function test
CBC
S electrolytes
أجت النتيجة ال
RBS = 345 mg/dl 🙂🙂
دزينة GUE، رجع النه
Ketonuria +++ 🙂🙂
Dx = diabetic ketoacidosis DKA
فهاي لو مو الروتين انفستكيشن، كلش صعب نعرفها. اصلا ال DKA ما كان ضمن قائمة ال DDX للأسف.
فلذلك تشوفون اخر امسكيوات نزلتها كلها ع RBS، فقط حتى تعرفون اهميتة.

والبيشنت عنده jaundice + direct bilirubin +elevated liver enzymes
وعدنا excluded لل drug hx & virology
لهذا نفكر ب
⏩ inherited liver diseases especially wilson disease(bc Cu is more toxic than Fe & it has earlier onset So excluded hemochromatosis in this case )
⏩ autoimmune hepatitis (bimodal distribution: 10–20 yo & 40–60 yo)
⏩ amyloidosis (rare case)
So do wilson and autoimmune hepatitis screen

Otherwise healthy, 30 yo M presented with palpitation since 30 min.
He had the attacks periodically, some attacks resolved spontaneously, some need medicines.
Most attacks are excercise-induced.
