مثل ما تشوفون ان
Measured PCO2 = 33
و المريض عندة metabolic acidosis.
ف اني لازم احسب شگد يصير respiratory compensation.
يعني هاي الـ acidosis، عند الشخص الطبيعي شگد لازم يقل الـ PCO2 و هذا يسمونه Expected pco2
شلون نحسبه؟؟؟
Expected PCO2 = 1.5 + Measured HCO3 + 8 +/- 2
و الحسابات اعلاه بالصورة و النتيحة كانت
14.5 - 18.5
بينما في الـ ABG كانت القرآة للـ PCO2 هي 33
يعني ما صاير Well compensation
بمعنى اخرى
This patient has relative retention of PCO2
يعني المفروض من هذا البيشنت يطلع الـ PCO2 اقل من ١٨ و اكثر مو ١٤
بينما هو باقي ٣٣
صح الـ ٣٣ هي اقل من الطبيعي الي هو 35-45، لكن هذا الطبيعي في حالات الـ Normal ph
اما في حالك ال metabolic acidosis فلازم الـ measured pco2 يطلع ضمن الرينج مال الـ expected حتى اكول عندة normal respiratory compensation.
Si this patient has poor compensation 👉👉 this pt has respiratory acidosis
من تقراه بالبداية تمول عليه هو respiratory alkalosis
بس بالحقيقه هو لا.

تگدر تنطي الـ Na bicarbonate دايركت خلال 5 دقائق.
الـ Na bicarbonate أحد أكثر العلاجات الي ما جاي ينصرفن بصورة صحيحة.

While LBB supplied by more distal branch 👉👉 smaller infarct size

اعتقد السستولك و الدايستولك حاليا قل استعمالهن حتى بالطوارئ.
المهم هو
Mean arterial pressure MAP = 2/3 DBP + 1/3 SBP
مشكلة الدايستولك و السستولك هو أنه يقيسن الضغط في specific cardiac cycle، و الي ممكن يختلف من سايكل لأخرى.
فهسه المهم هو الـ MAP الي يمثل BP as whole، بمعنى اخر الـ MAP يعكس الـ Reperfusion pressure و الي هو بصورة اساسية معتمد على الـ DBP (مثل ما موضح ب المعادلة اعلاة).
That is why elevated DBP predict hypertensive complications more than elevated SBP.
2 patients, one with BP 200/90 and the 2nd one has BP 180/140 👉👉 The 2nd one is more reliable to get hypertensive emergency like ACS or stroke than the 1st one, because he has higher MAP.
و لذلك عند الحوامل حتى نراقب الـ pre-eclampsia نكون معتمدين أكثر على DBP.

حسب normal range نبدا نقارن ونحسب كالاتي :
- pH < 7.35 👉 Acidosis
- pCO2 < 35 , pCO2 and pH change in the same direction 👉 metabolic disorder
- ↓ pH and ↓ pCO2 👉 metabolic acidosis
- HCO3 < 22 👉 metabolic acidosis
primary metabolic acidosis with secondary respiratory alkalosis why??
🔖 Compensation 👉↓ Arterial pH (↓ HCO3) 👉 ↑ stimulation of medullary chemoreceptors 👉 ↑ respiratory rate (hyperventilation) 👉 ↓ PCO2
بما ان هو metabolic acidosis نبدا نفكر ب anion gap حتى نحدد السبب وحسب المعادلة :
anion gap = (Na+K)–(CL+HCO3)
👉 (123.6+7.26)-(90+7) = 36.5 👉 high anion gap metabolic acidosis(HAGMA)
بعدها نحسب دلتا حتى نشوف هو pure or mixed حسب المعادله
delta ratio = (anion gap - 12) / (24 - HCO3)
👉 (36.5-12) / (24-7) = 1.4
اذن يعتبر
Only pure HAGMA is present or uncomplicated HAGMA.
Elevated anion gap causes:
- lactic acidosis (MC)
- ketoacidosis
- chronic kidney disease
- certain toxic ingestions
Treatment:
- ABC
- O2
- 2 amps bicarbonate
- 10 units IV Insulin
- 1 Calcium gluconate
- IV Insulin drip
- volume resuscitation, 0.9% NS (200-250 ml/hr)
- Monitoring (electrolytes, cbc, AbG)

Carvedilol has
1. Beta 1 blocker
2. Beta 2 blocker
3. Alpha 1 blocker
4. Calcium channel blocker
Beta 2 blocker + alpha 1 blocker + CCBs 👉👉 that is why carvedilol has vaso-dilator effect 👉👉 so the side effects like impotence, peripheral coldness, and easy fatiguiability are markedly low in this drug
5. Anti-oxidant effect
6. And it is the only beta blocker that increases heart rate as the dose increased.

Tips about beta blockers:
1. 1st generation (non selective) like propranolol and Nadolol
2nd generation (selective): atenolol, bisoprlol, metoprolol, and nebivolol 👉👉 the highest B1 selective is nebivolol, the 2nd one is Bisoprolol.
3rd gen (alpha and beta blockers) like carvedilol and Labetalol.
2. Bisoprolol (concor): has the lowest drug drug interaction profile, the most potent BB with renin inhibitory action (by up to 65%).
3. Nebivolol:
Has vasodilating effect via nitric oxide release 👉👉 so it is associated with less side effect of BBs like impotence, cold extremities, and fatigue.
Decrease the onset of DM, but not treat it.
4. Carvedilol:
Has alpha blockers activity 👉👉 peripheral vasodilitation 👉👉 reflex sympathetic hyper-Discharge 👉👉 so has less bronchospasm, less impotence, but paradoxical increase of the HR as the dose increases (the only BB that has this feature).
Maybe the only BB that decreases serum triglyceride level and increase the insulin sensitivity index (so it is the favoruble BB in DM).
Carvedilol and Nebivolol are the only BBs that have anti-oxidant effects, perhaps decreasing hyperglycaemia-induced tissues damage !!!!!
5. Labetalol:
Has B1, B2, and alpha 1 blocking activity + partial B2 agonist 👉👉 so it has - ve inotropes + vasodilator properities. That is why, it is one of the most preferred medications in HT emergency.

Tips about ARBs
1. Telmisartan and Olmesartan are the only ARBs that proved to have Insulin sensitizer action via PPAP gamma receptors.
2. Losartan is the only ARBs that used for stroke prophylaxis.
3. Valsartan and Candesartan are the best ARBs for heart failure.
4. Irbesartan and Losartan are the best ARBs for diabetic nephropathy.
5. Olmesartan is the only ARBs that has anti-Inflammatory action
6. The most effective ARBs for BP reduction is Olmesartan > Telmisartan > Irbesartan > Losartan > Valsartan.
7. The only ARBs that decreases serum uric acid is Losartan
8. The best time to take ARBs is bedtime, not just to reduce morning BP surge, but this time proved to reduce the chance of type 2 DM up to half (as some spanish researchers proved that). !!!!!!
9. Simply: if you want to choose ARBs for a patient:
A. Just HT 👉👉 u r free to choose anyone but the best is Telmisartan
B. HT with hx of TIA 👉👉 Losartan
C. HT + DM without nephropathy 👉👉 Telmisartan
D. HT + DM + nephropathy 👉👉 Losartan or Irbesartan
E. HT + heart failure 👉👉 Valsartan or Candesartan

This ecg for elderly F presented with fatigue and SOB.
She has HT, DM, hypothyroidisn, CKD, osteoporosis, lens transplant, and recurrent venous thromboembolism VTE.
O/E
Morbidly obese female, Puffy face, oriented, adentous, flapping tremor, terry nails, pitting edema of lower limb, normal JVP with preserved hepato-jugular reflex.
This ECG of the culprit patient.
