after 2months , bilateral symmetrical non prupritic skin rash developed in lower and upper limbs , not responded to anti histamine .
🖍no other drug history nor FHX of dermatological disease .

📌save patient life and sent for auto antibodies ( anti GAD65 ab , Anti IA 2 Ab , Anti ICA ab )

📌 52 years old female patient with diabetes mellitus on oral treatment, presented with bilateral foot and peri orbital edema .
📌 no Hx of CVD , nor Hx peripheral arterial disease.
📌 DM is poorly controlled, and no dietary restriction
✅ Examination : bilateral piiting leg edema grade III , vitally stable , mild peri ocular edema . soft abdomen.
✅ Investigation.. unremarkable RFT , LFT ,TFT ,CBC ,CRP ,ESR , abdominal Us .
but when we sent urinary albumin creatinin ratio interestingly, results as shown above .
🖍diabetic nephropathy is one of commonest nephropathy, when albuminuria is Hallmark of early damage.
🖍if expect that , do urinary albumin creatinin ratio test , that detect early renal insult.

A 57-year-old female patient with no significant past medical or surgical history presented with intermittent, diffuse, colicky abdominal pain persisting for several weeks, which did not resolve with antispasmodic medication.
🏥 There was no associated weight loss or anorexia.
On physical examination, the abdomen was tympanic and distended, with a painful, non-reducible swelling noted in the right inguinal region.
✅An erect abdominal X-ray revealed diffuse gaseous distension but no definitive signs of frank intestinal obstruction.
In summary, an inguinal hernia with a possible partial intestinal obstruction must be considered.

TSB 5g/dl .
heptoglobin too low
✅giving her acute attacks after drug initiation , neg FHx of G6PD or hereditary hemolytic anemia, so its a case of drug induced hemolytic anemia.
✅NSAID one of common drug causing drug induced Hemolytic anemia ( keep in mind)

60 yrs old male patient with Hx of diffuse colicky abdominal pain for 2months , dyspepsia, nausea, WT loss 7kg . normal bowel motion
neg FHX of autoimmune disease or malignancy .
PMHx .. poorly controlled DM
✅ on examination.. significant right lower abdomen tenderness .
✅ on colonoscopy.. significant ileal thickening with ulceration and stenosis , patulous ileocecal valve .

1. Crohn's ileitis ( but age in too old for diseae onset) .
2. chronic infectious ileitis ( TB ileitis, fungal ileitis).
3. drug induced ( NSAID induced ileitis )
هنا دزينا على
interferon gamma release assay ( IGRA) ... and was positive.
🖍 its case of TB ileitis. ( giving Hx of poor immunity ( poorly controlled DM , old age)) .
✅ Ileum is most common site for TB within GIT tract .
✅ clinical manifestations are variable, ranging from mild abdominal pain , to partial intestinal obstruction.
🖍 don't miss TB ileitis in any patient with ileitis and risk factor for 2nd TB reactivation.

78 yrs old male patient complained of black discoloration of cannula sites , after admission for GIT bleeding.
When we see blackish superficial necrotic tissue .. send for renal function test.
RFT .. S.creatinin 2.9mg/dl .. urea 280mg/dl .

Calciphylaxis, also known as calcific uremic arteriolopathy or “Grey Scale”, is a rare syndrome characterized by painful skin lesions.
It involves a buildup of calcium in small blood vessels of fat tissues and skin.
من الشغلات الشائعة عدنه للاسف : عدم قياس الضغط بالطريقة الصحيحة .. مما يؤدي الى overdiagnosis or under diagnosis of HTN
فمن الشغلات التي يجب مراعاتها :-
🔴 patient not smoke or take meal or caffein drinks for at least 30 min .
فلا تستعجل بالقياس بعد سؤال المريض .
🔴 Ask pt to empty bladder if has full bladder .
🔴 patient should be comfortable in sitting position .
بحيث يكون متجي ظهره .. ورجله على الارض اثنينهن .. مو كاعد رجل على رجل .
🔴 arm of patient should be at level of heart .
يعني يفضل يكعد على مقعد بي اسناد للايد .. مو يرفع ايده للاعلى دون ان يسندها على مسند .. لان رفعه للايد لمدة طويلة راح يزيد المجهود وبالتالي يرفع الضغط .. وهاي مفقوده للاسف بواقعنه .
🔴 blood pressure monitor must be validated .
للاسف هواي من الاطباء يستخدمون اجهزة غير معتمدة عالمية .. وهاي راح تطي قياس خاطئ .
🔴 cuff must be at least 2cm from cubital fossa

rare nice case ( Acrodermatitis Enteropathica)
6 yrs old thin child presented to us with chronic peri oral scaly lesion .. on further questionning there is also genital scaly lesion .
positive FHX of similar condition .
this is typical case of inherited Zinc deficiency, called acrodermatitis Enteropathica, when absorption of Zinc is impaired.
ميزته انو scaling spare upper lips margins ..
📌presentation is usually at the time of weaning in infants who are breastfed and earlier when they are formula-fed.
📌treatment is lifelong Zinc replacement.

76 yrs old female with Hx of primary biliary cholangitis for 1yr , presented with generalized body itching without skin lesion , despite on proper Rx 😀.
There is Weight loss , anorexia .
Review of system ... neg apart of dry eye , dry mounth , good urine output .
❌ on examination 👉 unremarkable, apart of congested red eye 👁, dry akin .
❌ investigation 👉 ESR 120 , ANA positive, anti La positive 🙄 .

Very interesting case finding .
22 yrs old female with Hx of bilateral lower limb weakness for 2 yrs duration , aggrevated last 3 months when patient got pregnant 🤰, and developed attacks of hyperemesis gravidarum , and condition complicated by upper limbs weakness .
No difficulty of breathing , no incontinence.
Patient was addmitted to hospital 3 wks ago , S.K was 1.4 , with resistant hypokalemia that coorected after magnesium infusion .
Patient got improved, and come for follow up regarding Accidental findings of high liver enzymes (AST 540 , ALT 121) .
Our assessment 👉 chronic weakness, aggrevated recently, low k , low Mg , high liver enzymes , but AST >>> ALT .. let's think about myopathy...
S.CPK was sent .. result 8000 😳
Positive FHX of similar condition in aunt , undiagnosed, with intermittent weakness ..
DDx :Familial hypokalemic periodic paralysis, renal tubular acidosis.
We did neurological consultation.. Dx 👉 Familial hypokalemic periodic paralysis 🙏🌹
