What Determines the Stability of the Gingival Margin After Root Coverage? When treating gingival recessions in the esthetic zone, it is important to evaluate not only the depth of the defect and the ability to advance the flap coronally. For long-term stability, the following factors are essential: 🔹 soft tissue thickness; 🔹 width of keratinized tissue; 🔹 flap stability; 🔹 quality of graft fixation; 🔹 tension-free flap closure. A connective tissue graft should not be used by default. It is indicated when there is a need to improve the biological conditions for healing: to increase tissue thickness, support the gingival margin, and reduce the risk of recurrent apical displacement. 🦷 A fractured instrument in the root canal is not always an indication for immediate removal. What matters far more is determining which approach will truly improve the tooth’s prognosis: removing the fragment, bypassing it, or avoiding aggressive intervention altogether. Today, endodontics relies increasingly not on technical skills alone, but on a precise analysis of the clinical situation. Does smoking always mean a poor prognosis for treatment with the All-on-4® concept? Smoking is widely recognized as a risk factor in implant dentistry. But does it primarily affect implant survival, or does it have a greater impact on long-term bone stability? In a 5-year retrospective study, Dr. Paulo Malo and colleagues compared the outcomes of 100 smokers and 100 non-smokers who underwent full-arch mandibular rehabilitation using the All-on-4® concept. The findings were more interesting than expected. 🦷 Implant survival rates remained high in both groups:

However, the results were different when marginal bone loss was evaluated:

In endodontics, overly aggressive instrumentation rarely looks like a mistake in the moment. On the contrary, it may feel like the clinician is making the treatment more reliable: enlarging the canal more thoroughly, improving irrigant access, and creating a more predictable shape for obturation. But the clinical line between adequate shaping and overtreatment can be very thin.
