#NOTE 2 ■How can you stop bleeding? ☆To stop Bleeding,you need : Empty,Contracted,Retracted Uterus هسه اذا بعدنا بال ● Assessment of severity of bleeding(نقطه 2) ولكينا 🌹SEVERE》Termination مالك علاقه بالfetus عايش ميت bec. you may lose the Pt. in few minutes.. فه هسه شون اوكف الbleeding?? 1.Empty uterus》by Termination either vaginal delivery or C/S 2.Contracted & Retracted uterus 🌹Not SEVERE》 هسه اذا البليدنك مو كلش severe حسب ال Gestational age وال viability of fetus يعني نقطه 4 و5 ▪️APH +mature fetus (37weeks and above)..Termination APH with negative fetal viability .. Termination APH with gross fetal anomalies .. Termination هسه اذا وحده 🌹Not severe and pre term (about 34 week) (not mature)..expectant يعني whatchful monitoring ننتظرها شوف شتصير اذا صار البيبي تيرم>termination او يجوز تصير severe او يجوز يموت البيبي >termination #Gyn_obs

#Note1 من تجيك مريضه antepartum bleeding شتسويلها؟ ■1.لازم تسويلها addmission even slight vaginal bleeding لان انته ورة ساعه متعرف شراح يصير بيها وبعدين؟؟ ■2.Assessment of Severity..Check the vital signs BP,PR,RR,Temp شتستفاد منها؟ Pulse Rate: ♤Tachycardia..significant blood loss(Hypovolemic shock) ♤Normal..not significant to the level that stimulate sympathetic system ♤Bradycardia..very serious condition ,the pt may enter in Cardiogenic Shock. Blood Pressure: ♤Hypertensive..one of risks factors of APH ♤Normotensive ♤Hypotensive..indicate significant blood loss. 》Also you assess consciousness of pt those with cardiogenic shock are agitated,pale and then become semi-conscious. ●sometimes the pt. comes with slight vaginal bleeding تباوع عليها تشوفها pale ,Tense Abd., Tachy ,Hypotension وفايته ب shock bec.some times blood clot so bleeding concealed within uterine cavity. ■3.put 2 IV lines.,Foleys catheter ■4.Assessment of Gestational age by LMP,US,Examination. ■5.Assess the viability of Fetus 》اذا عرفت من الLMP او بالUS انو ال gestational age طلع 37W,+Heart Beat,fetus is mature شتنتظر بعد علما تشوك المريضه؟! سويلها 》Termination 》اذا الHeart beat negative هم سويلها termination هوة الفيتس ميت شتنتظر بعد! 》 اذا premature + fetal heart negative + mild bleeding .. هذا نسويله conservative ننتظرها تصير ستيبل بعدين نشتغل #Gyn_obs
