Pneumoperitoneum طفل و جاي vomiting and abdominal distention يا اخي خلي أيدك عليه و افحصة قبل لا تنطي مثبريم شراب براسيتامول تحاميل. ال examination اخذناه حته نحجيه للدكتور لو حتى نطبقة ع المرضى. مرات الطفل يكون صعب تحصلة Erect chest X ray, ف بهجي حالات سوي left lateral decubitus abdominal X ray و مثل ما واضح أمامكم ال pneumoperitoneum. #pediatric

Pus some where, pus no where, is the pus under diaphragm. يجي البيشنت عندة hectic fever and toxic, ف انت تحلف عليه عنده pus some where. تسأل عن ال localizing symptoms مال ال pus, يعني cough إذا lung abscess, أو abdominal pain إذا peritoneal abscess و هكذا. بس هذا البيشنت ما عنده localizing symptoms, فگالوا عنه pus no where، يعني بحثنه عن ال site of pus clinically بس ما لگيناه In these case (pus some where and pus no where), think about pus under diaphragm. زين ليش ما عندة localizing symptoms, لأن Superior Sub phrenic space (between diaphragm and liver) is not covered by peritoneum (bare area of liver), so the pain not seen initially but seen later on When the liver capsule (Glisson capsule) is irritated. فمن يجي عنده pain و تسويلة Erect CXR ممكن تشوف Air under diaphragm و ممكن يُشخص خطأً ع انه perforated viscus. #surgery

Hypovolaemia vs dehydration HYPOVOLEMIA refers to any condition in which the extracellular fluid volume is reduced, and results in decreased tissue perfusion. It can be produced by either salt and water loss (e.g. with vomiting, diarrhea, diuretics, or 3rd spacing) OR by water loss alone, which is termed DEHYDRATION. Salt + water loss comes primarily from the extracellular fluid whereas pure water loss (dehydration) come from the total body water, only about 1/3 of which is extracellular. Thus for dehydration to produce the same degree of extracellular volume depletion as salt+water loss, 2.5 times as much fluid needs to be lost. Patients with DEHYDRATION are always hypernatremic.
