#Emergency #ENT ⭕️ Epistaxis ايضا ما يعرف بال Nose bleed ✅ خل نجي نشوف شنو اهم الاسباب causes of epistaxis Digital trauma (Most common cause) Dry air Chemical irritants like steroids Systemic factors increasing risk of bleeding Chronic nasal cannula use Rhinosinusitis Neoplasia Deviated septum شنو الاجراءات اللي نسويها من يجينا شخص للطوارئ عنده epistaxis؟ Direct pressure اول شيء لازم نسويه عادة والضغط يكون على الجزء ال cartilagenous مو ال bony علمود يكون للضغط effect Chemical cauterization is best next step اول خطوة تسويها اذا مفاد ال direct pressure 0.05 oxymetazoline with 4% lidocaine are applied then we apply  Silver nitrates ال sliver nitrate لازم ينحط على جهة وحدة من ال septum لانه ممكن يسوي septum perforation ولهذا مو كلش يفيدنا اذا ال source of bleeding bilaterally بعد هالخطوة اكو خطوات مو كلش متوفرة ال supply مالتها بمستشفياتنا منها ال fibrogenous materials وال Tranxemic acid Surgicel (hemostatic material) if failed chemical cauterization or in place of nasal tampons بالنسبة لل Tranexamic acid وشلون نستعمله Tranexamic acid 200mg direct Or 500mg with 5cc NS Or 500mg cotton pledgers soaked بالاماكن الما متوفرة الشغلات الفوك نروح لل anterior nasal packing Anterior nasal packing Used if direct pressure, vasoconstrictors  or chemical cautery is not successful and thrombogenic materials are not available Use bilateral anterior packing for optimal pressure طريقة ال anterior nasal packing تكدرون تشوفوها باليوتيوب Posterior nasal packing شوكت نلجأ اله؟ Uncontrollable anterior bleeding and need for posterior packing are indications for ENT consultation Anterior packing that will removed in less than 24 to 36h are not needed to have Antibiotics اي مريض يحتاج posterior packing يحتاج ادمشن للمستشفى Posterior packed patients needs admission طبعا ال dx of posterior bleeding هو by exclusion Posterior packing can be used with using 14F catheter في بعض الحالات اكو طريقة خاصة انه نستعمل الفولي كاثيتر كطريقة لل posterior packing ✅ اكو علاقة بين ارتفاع الضغط وال epistaxis؟ No clear relationship exist between elevated BP and Epistaxis

#Emergency ⭕️ Heat emergencies ✅ بما انه داخلين على صيف لاهب 🔥 ضروري نحجي بهيج موضوع Heat emergencies are spectrum of disorders related to exposure to high environmental temperature مثل ما كلنا هي spectrum يعني اكو بيها mild disorder وهمين اكو بيها severe life threatening conditions ✅ بصورة عامة تشمل ال Heat emergencies الاتي Heat edema Heat prickly Heat cramps Heat stress (Exhaustion) Heat stroke اول 2 conditions يعتبرون فد شيء benign ✅ Heat edema can occure due to peripheral vasoconstriction and tend yo be in dependent body area ✅ Heat exposure also can lead Heat prickly (Miliria) which is a skin rash caused by trapped sweat under skin واذا سامعين بيه اهل قبل يسموه (حصف) خصوصا بالاطفال الصغار ويكولون عليه يصير بالحر .. طبعا علاج هالحالتين سهل For edema, try to lower body temperature and elevate dependent body area For rash also cool the skin, you can use calmine lotion. هسة نجي لل 3 conditions الباقية ✅ Heat cramps are muscle cramps usually occuring in calf muscles and are caused by electrolytes imbalance وعلاجها هو بتعويض السوائل والاملاح ... ✅ Heat stress or exhaustion have same symptoms of muscle cramps plus normal or elevated body temperature (less than 40) plus, headache, nausea, vomiting, dizziness, malaise and orthostatic hypotension ✅ Heat stroke include same presentation with heat stress but with more high temperature (>40) and also neuroogical manifestations like altered level of consciousness and seizures طبعا العلاج هو supportive و lowering body temperature افضل طريقة نخفض بيها درجة حرارة الجسم هية انه نبلل جسم المريض ونخلي المي يتبخر وبالتالي تنخفض درجة حرارة الجسم (مشابهة لحد ما طريقة عمل الكمادات) واذا تنتبهون هاي الطريقة ديستعملوها حاليا بال prevention مثلا بالحج او بالزيارات المكتضة او الحشود وهو يحطون مراوح وبيها سستم يضخ المياه .. هي هاي الفكرة الاصلية من هالسستم انه يساعد على خفض درجة حرارة الجسم ... ✅ Heat stroke is top emergency and can lead to many complications mentioned below ✅ Also do not midiagnose heat emergency as there is several ddx especially in older age group.
