❌ Why Rheumatic Fever (RF) usually doesn't occur after antibiotic treatment: RF is a type II hypersensitivity reaction — it involves molecular mimicry between streptococcal M proteins and human tissues (like heart valves, joints, etc.). Unlike PSGN, RF requires a sustained immune response triggered by ongoing exposure to the live bacteria. If you treat strep pharyngitis within 9 days of symptom onset, it usually prevents RF because you eliminate the bacteria before the autoimmune response gets going. ➡️ So, early antibiotic treatment breaks the cycle before your immune system starts attacking your own tissues.

💡 Why Post-Streptococcal Glomerulonephritis (PSGN) can occur even after treatment of strep pharyngitis: PSGN is caused by immune complex deposition in the glomeruli — not by direct infection. Even if the bacteria are killed by antibiotics, the immune system may have already created antibodies that form complexes with streptococcal antigens. These antigen-antibody complexes get trapped in the glomeruli and trigger inflammation. This immune reaction typically happens 1–3 weeks after the initial infection. ➡️ So, by the time treatment starts, the immune system may have already been primed, and the damage is due to the immune response, not ongoing infection.
