🩸 Hémorragie digestive : À connaître absolument: Hématémèse ➡️ Rejet de sang PAR LA BOUCHE, lors d'un effort de vomissement. Méléna ➡️ Selles noires, goudronneuses et nauséabondes = sang digéré. Rectorragie ➡️ Émission par l'anus de sang rouge non digéré. 💡 Clinique = toujours préciser : couleur du sang + voie d'élimination + contexte. #gastroreminder

🩺 DIGESTIVE SEMIOLOGY — CLINICALS QUICK REVIEW : 🔹 Dysphagie & Vomissements : Dysphagie = difficulté / gêne à la déglutition. Vomissement = rejet actif et brusque par la bouche du contenu gastrique, souvent précédé de nausées. ⚠️ À différencier de : Régurgitation → remontée passive, sans effort de vomissement. Rumination / mérycisme → remontée d'aliments déjà ingérés. 🚩 Ce que la nature du vomissement peut orienter : Alimentaire tardif / vomissement de stase → obstacle à l'évacuation gastrique. Bilieux → obstruction située sous l'abouchement des voies biliaires. Hématique → hématémèse. Fécaloïde → évoque une occlusion intestinale avancée. En fusée, sans nausée → penser à une hypertension intracrânienne ou un syndrome méningé. #gastroreminder

#Neuro 🔥 Signe de Hoffmann. • L’examinateur tient le majeur du patient et réalise une flexion brusque de sa phalange distale, puis observe la réponse des autres doigts. 🔴 Hoffmann positif : Une flexion involontaire du pouce et de l’index apparaît. ببساطة: إذا تنبّه الإصبع الوسطى وحدثت حركة لا إرادية للإبهام والسبابة، يكون signe de Hoffmann positif. 🧠 Il s’agit d’un signe de libération pyramidale du membre supérieur, pouvant notamment être retrouvé en cas de myélopathie cervicale ou d’atteinte du faisceau corticospinal. ⚠️ Attention : ce signe n’est pas spécifique et peut être présent chez certains sujets sains. Il doit toujours être interprété avec le reste de l’examen neurologique.
