⚜️ Cervial Lesions .
🔹المحاضرة الرابعة اليوم
🔹 وقت المحاضرة ساعة ونص (9:30-11:00 ).
🔺Total Necrosis of the Pulp
🔆Coagulation Necrosis: The cell shape is still recognizable under a microscope, but the internal details are gone.
Liquefaction Necrosis: The entire cell structure has disappeared, leaving a liquefied area.
🔆Treatment: The only options are root canal treatment or tooth extraction.
🔺 Acute Pulpitis Superimposed on Chronic Pulpitis
🔆 This occurs when a long-standing chronic condition suddenly flares up into an acute, painful attack.
🔆 Severe pain, particularly at night.
🔆 Relief Mechanism: Pain decreases slightly once an abscess forms, but full relief only occurs through drainage and Antibiotics followed by a root canal or extraction.
🔺 Chronic Partial Pulpitis with Partial Necrosis
🔆 The Cause: Occurs when an irritant persists, leading to "liquefaction" or "coagulation" necrosis in the deeper pulp.
🔆 Spontaneous pain.
Can last for about 30 minutes, does not respond to painkillers (analgesics).
🔆 Diagnosis: Requires a radiograph (X-ray).
It can be "open" (visible caries) or "closed" (caused by toxins).
🔆 Treatment: Either a root canal or tooth extraction.
🔺Chronic Total Pulpitis with Partial Necrosis
🔆 The Cause: The inflammation spreads to involve the entire pulp (both the crown and the root portions).
🔆 Severe pain that can last for several hours.
Throbbing pain that worsens when the patient lies down to sleep due to increased internal pressure.
🔆 Treatment: Like the partial form, this is irreversible and requires a root canal or extraction.
🔺Chronic Partial Pulpitis (Without Necrosis )
🔆 Causes: Deep caries, pulp exposure, operative procedures, deep periodontal lesions, or orthodontic movement.
🔆 Location: The inflammation is confined to the coronal portion.
🔆 Pulp Polyp (Chronic Hyperplastic Pulpitis)
Who gets it: Primarily children and young adults due to their rich blood supply and strong immune response.
Appearance: A specific type of tissue growth that bleeds easily when touched.
🔆 Diagnosis: It can be distinguished from a gum polyp by using a dental probe.
🔆 Conservative: Vital pulpotomy (removing only the top pulp) can be successful if there is no necrosis Or Root canal therapy or extraction.
🔆 Classification:
Reversible: If no partial necrosis exists.
Irreversible: If partial necrosis has begun.
🔺Acute Pulpitis
هذا ملخص للمعلومات حتى يصير الحفظ أسهل :
🔆 Cell Damage: Odontoblast cells may rupture due to edema (swelling).
Timeline: This reaction typically lasts 3 days.
🔆Pain: Patients experience mild pain triggered by hot or cold stimuli.
🔆 Duration: The pain lasts only as long as the stimulus is present.
🔆 Classification: This is categorized as reversible pulpitis.
