Upon entering the flat, the police discovered the man in an unusual and unsettling position. He was found squatting on the floor, wearing a full neoprene diving suit. Chronic pain management became a necessity for the patient. Eventually, it was discovered that she had developed a multidrug-resistant Pseudomonas wound infection.
In the absence of other effective treatment options, the surgeon decided to experiment with a porcine urinary bladder matrix (UBM) product. Remarkably, just three days after the application of the UBM product, the patient reported significant pain relief, no longer requiring opioid medications. The wound care regimen included weekly, painless applications of UBM powder matrix and sheet epithelial basement membrane on top of the powder, followed by a covering of lubricating jelly and Vaseline gauze. This dressing was secured with plain gauze and tape, and the patient wore it for several days without changing it, returning weekly for dressing changes.

After just two weekly applications, the patient's infection had completely resolved, and she began to grow islands of new epidermis over her chronically open mastectomy wound. By three months, the wound had started to contract, and four months after the initial placement of UBM, the wound had significantly decreased in size to approximately 3 cm by 2 cm by 2 cm. The patient's pain had fully resolved, and new epithelial islands were covering the granulation tissue in the wound.
This case underscores the potential of porcine urinary bladder matrix in managing complex, non-healing wounds, particularly in patients with significant comorbidities and multidrug-resistant infections. The success of this treatment in resolving infection, promoting wound healing, and alleviating pain offers a promising alternative for similar challenging cases in wound management.
