📌 INSULIN adverse effects:
🔹 1) Hypoglycemia — Most important & most common
Why?
• Too much insulin
• Skipping meals
• Excessive exercise
• Injection into exercising limb (↑ absorption)
Symptoms
• Sweating, tremor, hunger
• Palpitations
• Confusion, seizures, coma (severe)

🔹 2) Weight Gain
Due to:
• ↑ fat storage
• ↓ glucosuria
• Overcorrection of hypoglycemia with food
Most common with premixed & basal–bolus regimens.

🔹 3) Lipodystrophy at Injection Site
A) Lipohypertrophy (most common)
• Fatty lumps → ↓ absorption of insulin
• Caused by repeated injections at same site
B) Lipoatrophy (rare, immune mediated)
• Loss of fat at injection site
• Due to antibody reactions (older insulins)

🔹 4) Injection Site Reactions
• Pain
• Redness
• Swelling
• Itching
More common with impure or improperly stored insulin.

🔹 5) Allergic Reactions
A) Local allergy
• Swelling & itching at injection area
B) Systemic allergy (rare)
• Urticaria
• Angioedema
• Anaphylaxis (very rare)

🔹 6) Hypokalemia (Important)
Insulin drives K⁺ into cells → can cause:
• Muscle weakness
• Arrhythmias
• ECG changes
Seen especially when insulin is given IV (e.g., DKA).

🔹 7) Edema
“Insulin edema”
Occurs when:
• Starting high-dose insulin
• Rapid improvement in glycemia
Caused by salt and water retention.

🔹 8) Rebound Hyperglycemia (Somogyi effect)
Post-hypoglycemia exaggerated hormonal response → morning hyperglycemia.

The CURB-65 score is a clinical prediction rule used to assess the severity of community-acquired pneumonia and predict mortality risk. It is based on five criteria, each assigned one point:
📊 CURB-65 Score
• 🧠 Confusion — new onset
• 💧 Urea (BUN) > 7 mmol/L (≈ 19 mg/dL)
• 🌬️ Respiratory rate ≥ 30 breaths per minute
• 🩺 Blood pressure: systolic < 90 mmHg or diastolic ≤ 60 mmHg
• 🎂 Age ≥ 65 years
Scoring & Risk Stratification
• 0–1 points: Low risk ➤ suitable for outpatient treatment
• 2 points: Moderate risk ➤ consider short hospital stay or close outpatient monitoring
• 3–5 points: High risk ➤ hospitalization, possible ICU for 4–5
As the CURB-65 score increases, mortality risk rises — higher scores correlate with greater need for aggressive management.

🦴 Explanation of the Image
🔹 Intracapsular fractures:-
These occur within the capsule of the hip joint (the articular capsule surrounding the femoral head and neck).
1. Capital fracture – involves the head of the femur.
2. Subcapital fracture – just below the head, at the junction between head and neck.
3. Transcervical fracture – through the mid-neck of the femur.
4. Basicervical fracture – at the base of the neck, where it meets the trochanteric region.
➡️ Clinical importance: These fractures are at high risk of avascular necrosis (AVN) and nonunion, because the retinacular arteries from the medial circumflex femoral artery supplying the femoral head may be disrupted.Often needs arthroplasty (replacement).

🔸 Extracapsular fractures:-
These occur outside the joint capsule.
1. Intertrochanteric (or transtrochanteric) – runs between the greater and lesser trochanters.
2. Subtrochanteric – just below the lesser trochanter, within ~5 cm distal to it.
➡️ Clinical importance: These fractures have better blood supply (no risk of AVN) but may be unstable due to muscle pull and comminution.Often internal fixation (nail, plate)

• Clinical signs of proximal femoral fracture:
• Affected leg appears shortened and externally rotated.
• Common in elderly females with osteoporosis after a minor fall.

💊 ACE Inhibitors — Common Side Effects
1. 😷 Dry persistent cough → due to ↑ bradykinin buildup in lungs
(resolves after stopping the drug)
2.⚡ First-dose hypotension → sudden drop in BP, especially if on diuretics or dehydrated
3. ⚠️ Hyperkalemia → ↓ aldosterone → K⁺ retention → monitor serum potassium
4. 💧 Renal impairment → ↓ GFR (esp. in bilateral renal artery stenosis)
(Check creatinine before and after starting)
5. 😵 Dizziness / Fatigue → from vasodilation and lower BP

🚨 Serious / Rare Side Effects
1. 😨 Angioedema → swelling of lips, face, tongue, airway (↑ bradykinin)
Potentially life-threatening
2. 🧫 Neutropenia / Agranulocytosis → rare (mostly with captopril)
• Risk ↑ in renal impairment or autoimmune disease
3. 💦 Proteinuria → uncommon, especially with high-dose captopril
4. 🚼 Teratogenicity → contraindicated in pregnancy ❌
• Causes fetal renal failure, oligohydramnios, skull defects

The triad of fever, right upper quadrant (RUQ) pain, and jaundice is known as “Charcot’s triad”.
This triad is classically associated with acute cholangitis, an infection of the bile ducts usually caused by bile duct obstruction, often due to gallstones, strictures, or tumors.
If hypotension and altered mental status accompany this triad, it indicates “Reynolds’ pentad”, signifying severe cholangitis and sepsis.
