A 6-week-old premature infant is admitted because of persistent jaundice, progressive abdominal distension, and poor weight gain. He had a prolonged NICU stay due to respiratory distress syndrome and received total parenteral nutrition for 3 weeks. During the neonatal period, he also developed severe indirect hyperbilirubinemia secondary to G6PD-related hemolysis and required exchange transfusion. The mother now reports pale intermittent stools and dark urine for the past 10 days. On examination, the infant is icteric with mild hepatomegaly but no splenomegaly. Laboratory tests show total bilirubin 16 mg/dL, direct bilirubin 10 mg/dL, elevated GGT and ALP, and mildly prolonged PT. Ultrasound reveals mild dilatation of the extrahepatic bile ducts with echogenic intraductal material, while the gallbladder is present and contracts after feeding. HIDA scan demonstrates delayed biliary excretion with partial passage into the intestine after 24 hours. Liver biopsy shows canalicular cholestasis without bile duct proliferation or significant fibrosis. What is the most likely diagnosis?

A 38-week male neonate is admitted on day 4 of life with poor feeding, lethargy, and progressive jaundice extending to the lower extremities. Laboratory evaluation reveals severe unconjugated hyperbilirubinemia with evidence of hemolysis. Despite intensive phototherapy, the total serum bilirubin continues to rise and approaches the exchange threshold. Double-volume exchange transfusion is performed successfully.
Six hours after the procedure, the infant appears clinically improved. Neurologic examination is normal. Repeat bilirubin level shows a marked decline to a value below the exchange threshold and below the intensive phototherapy threshold for age. Hemodynamics are stable, feeding improves, and hemolysis has not completely resolved.
