A 19-year-old male presented with a 3-day history of severe headache, fever, nausea, and progressive neck stiffness. One week earlier, he had been swimming and diving in a warm freshwater lake during a summer outing. Two days after swimming, he noticed a complete loss of smell, which was not associated with nasal congestion or trauma. His symptoms worsened, and he developed photophobia, vomiting, and increasing confusion. On examination, he was febrile, drowsy, and had marked nuchal rigidity with positive meningeal signs. Can you guess the cause? This is rare tricky OSCE question
