Donna, aged 12 years, is brought to you by her mother, Jane. Jane reports that Donna is becoming aggressive at school, and her grades have started to drop. Donna’s teachers contacted her parents to say they are concerned about her learning, and her behaviour cannot continue to be tolerated. Donna is in grade six at primary school. She is of normal height and weight and has no history of illness or surgery. She lives at home with her mother, her father (Tom) and brother (Michael). The SIRS is now in operation in Australia.2 The SIRS guidelines state that any incident involving non-consensual sexual contact is reportable (www.agedcarequality. gov.au/sites/default/files/media/sirs-guidelines- october-2021.pdf; chapter 3). Therefore, this incident is reportable. From 1 April 2021, all RACFs are required to have an incident management system in place. This system ‘enables their appropriate and timely prevention, identification, and response to all incidents’and requires that staff enter a series of mandatory details . This will involve entering details of both patients (the ‘perpetrator’ and the ‘victim’). Note that while it is mandatory for staff to report this incident, other people may also report an incident through the SIRS. #SIRS_Report #Serious_Incident_Response_Scheme
