Deficit knowledge Nursing diagnosis of pulmonary edema : 1. Impaired gas exchange ➡️ mainly oxygenation 2. Fluid volume excess 3. Activity intolerance 4. Anxiety Nursing diagnosis in spinal cord injury : 1.Impaired Gas Exchange related to muscle fatigue and weakness. 2.Decreased cardiac output related to venous pooling of blood and immobility as evidenced by hypotension and tachycardia 3.(constipation) related to location of fluid intake, diet and immobility↓injury, 4.Urinary retention related to injury and limited fluid intake as evidenced by decreased output and bladder distention 5.Impaired skin integrity related to immobility and poor tissue perfusion Nursing diagnosis of angina 1. Acute chest pain related to myocardial ischemia. 2. Activity intolerance related to myocardial ischemia. 3. Anxiety related to the severity of the disease 4. Deficient knowledge related to lack of previous exposure Nursing diagnosis of MI 1.Acute pain related to decrease myocardial oxygen supply. 2. Decreased cardiac output related to myocardial ischemia. 3. Anxiety related to fear of death/critical care 4. Deficit knowledge related to the illness and its impact on environment. 5. ⭐ زيادات محاضرة pulmonary edema ▪️ طريقين عشان اعرف بيهم نسبة الأكسجين  : • لما بحط العيان pulse oximeter ➡️ O2 saturation less than 80% • واسحب عينه ABG هلاقيها o2 ➡️ Less than 50% • وهلاقي العيان عنده respiratory alkalosis ومعناه العيان عنده hyperventilation بياخد نفسه كتير وان PH عالية • لما بجي اعمل auscultation the chest ⬅️ بسمع صوت crackles • Auscultation the heart : ➡️ S3 heart sound بسمع ومعناه أن في مشكله في ال LV ▪️ازاي اقلل pre load : • اقعد العيان في upright position • تاني حاجة اعطيه diuretics عشان تقلل ال fluid excess • بعطي V.d 👉 nitroglycerin ▪️ ازاي اقلل ال after load : • Anti hypertensive agents بعطيه أدوية بتظبط ضغط العيان واشرههم الmorphine بيقلل ال stress and anxiety اللي بيزودا الضغط

⭐ GIB Notes • main cause for upper GIB ▶️ Gastric Ulcer • main cause for lower GIB ▶️ Diverticulosis • اللي بيحدد cause and treatment ▶️ Endoscopy • لما يحصل hypovolemic shock ▶️ activation sympathetic nervous system 💥 UGIB ➡️ proximal to treitz ligament 💥 LGIB ➡️ Distal to treitz ligament 💥 Vericeal : end stage liver disease 💥 Non vericeal : peptic ulcer disease • 10% mortality rate associated with AGIT BIeeding • Upper GIB is 4 times as common as Lower GIB • بيحصل Upper GIB بنسبة 75 - 80 % • بيحصل peptic ulcer بنسبة 40 - 60 % من UGIB • بيحصل Esophageal varices بنسبة 10 - 25 % • الناس اللي عندها liver cirrhosis 50% منهم بيحصل ليهم Esophageal varices • لو الشخص فقد 15% من الدم بتاعه بيظهر عليه اعراض Tachycardia , orthostatic hypotension , syncope • لو الشخص فقد دم بنسبة 40% بيحصل له hypotension , hypovolemic occur , decrease perfusion of brain • pt with Hemodynamic instability ➡️ greater than 2 packed red blood cells • pt with coagulation , vitamin k can be given 10 mg IM , IV SLOWLY • بيحصل Diverticulosis بنسبة 30-50% في Lower GIB • peptic ulcer disease ➡️ causes by H. Pylori infection , Aspirin , NSAIDs • Stress Related Erosive syndrome ➡️ common in critically ill pt , more numbers ,shallower , more diffuse , prolonged stay in icu • Angiodysplasia ➡️ small vascular malformation in gut , aorta enteric fistulae • Mallory Weiss Tear ➡️ distal esophagus , causes by forceful vomiting , retching • Assess signs of shock ➡️ Cold extremities, tachycardia, hypotension , Chest pain, confusion, delirium, oliguria, rapid weak pulse 💥 Skin changes ➡️ If cirrhosis ➡️ palmer erythema, spider nevi ➡️ If bleeding disorders ➡️ purpura , echymosis ➡️ If coagulation disorders ➡️ hematoma , haemarthrosis, • Signs of dehydration ➡️ (dry mucosa, sunken eyes, skin turgor reduced). • Signs of a tumor ➡️ (nodular liver, abdominal mass, lymphadenopathy) 💥 Heamatemesis bright red or coffee Vomiting 💥Hematocaziea bright red stool 💥Melana black tray stool
