🌹 Iron Deficiency without Anemia (IDWA) vs Iron Deficiency Anemia (IDA)🌹 🌹 Step 1️⃣: Hemoglobin Level ▶️ IDWA : Hb NORMAL ▶️ IDA : Hb LOW 🌹 Step 2️⃣: Evaluate Iron Studies 🔵 IDWA 🔸Ferritin ↓ 🔸Serum iron ↓ 🔸TIBC ↑ 🔸Transferrin saturation ↓ 🔸MCV → Normal or slightly ↓ 🔸RDW → Normal or mildly ↑ 👉 Interpretation: • Iron stores depleted • Erythropoiesis still maintained • No anemia yet 🔴 IDA 🔸Ferritin ↓↓↓ 🔸Serum iron ↓↓ 🔸TIBC ↑↑ 🔸Transferrin saturation ↓↓ 🔸MCV ↓ (Microcytosis) 🔸MCH ↓ (Hypochromia) 🔸RDW ↑↑ 👉Interpretation: • Iron deficiency severe enough to impair RBC production • Anemia is established 🌹 Step 3️⃣: Peripheral Blood Smear 🟡 Iron Deficiency without Anemia (IDWA) • Mostly normal red blood cell (RBC) morphology • Occasional Microcytes or Ovalocytes • RBC size and hemoglobin content are generally preserved • No significant Anisocytosis 👉 Findings reflect early iron depletion without impaired erythropoiesis. 🔴 Iron Deficiency Anemia (IDA) • Microcytic Hypochromic red blood cells • Anisocytosis (↑ RDW) • Possible Pencil cells (elliptocytes) • Increased variation in RBC size and shape 👉 Findings indicate impaired hemoglobin synthesis and established anemia. 🌹 Step 4️⃣: Clinical Presentation 👉Summary 🔸IDWA : Low Ferritin + Normal Hb 🔸IDA : Low Ferritin + Low Hb 🔸Key Teaching Point Ferritin decreases ⬇️ first → Hemoglobin decreases ⬇️ later
