ملاحظات ذكروها بعض الاساتذة بالسكل لاب : رسبايرتري الاصوات المهمة Coarse crackles Fine crackles Wheeze وممكن يجي Absent sound on one side اذا كان الصوت bronchial breathing وانت كلت vesicular مراح تتحاسب لكن اذا كان vesicular وكلت bronchial هنا راح تتحاسب (تتنقص من الدرجة)

ملاحظات ذكروها بعض الاساتذة بالسكل لاب : كارديو الاصوات المهمة : AS AR MS MR وممكن يجي TR VSD اذا ال S1 و ال S2 كانت abnormal وانت كلت normal ما تتحاسب لكن اذا كانت normal وكلت abnormal or loud or faint تتحاسب ال VSD و ال TR هم pansystolic murmur يشبهن ال MR بس ما بيهن Radiation لل axilla عكس ال MR , وينسمعن افضل بال Tricuspid area وللتفريق بينهن (بين TR & VSD) نطلب من المريض ياخذ نفس عميق واذا زاد ال murmur يعني TR واذا ما زاد يعني VSD دكتور علي دريد ذكر انه ممكن بال TR او ال VSD تسمعون صوت بال axilla بسبب الدمية بيها مشكلة لكن شلون تميزون هذا الصوت فعلا radiation of MR او لا ؟ كال لاحظو الصوت اذا اكو صوت عالي بال Mital area اعلى شي هنا وبال axilla عالي بعلو الموجود بال Mitral area هذا يعني radiation of MR اما اذا اكو صوت بال axilla خفيف مو بعلو ال Mitral area فمعناها هذا مو radiation

Mitral regurgitation  Normal s1 normal s2 No added sounds No extra heart sounds Pansystolic murmur best heard in the mitral area Blowing in character radiating to the axilla Grade 4 Ddx: MR TR (increases with inspiration (Carvallo’s sign)) VSD (hand grip maneuver) Aortic stenosis Normal s1 normal s2 No added sounds No extra heart sounds Ejection systolic murmur best heard in the aortic area Harsh in character Radiating to the carotid artery Grade 4 Ddx: AS Aortic sclerosis (no radiation to the carotid a.) hypertrophic obstructive cardiomyopathy (increases with valsalva maneuver or squatting to standing) PS (there’s radiation to the left clavicle or shoulder) ASD (there’s a fixed splitting of S2) Mitral stenosis Normal s1 normal s2  No added sounds No extra heart sounds Mid diastolic murmur best heard in the mitral area Rumbling in character No radiation Grade 3 Ddx: TS (doesn’t increase with turning to the left lateral position) Maneuver: turn the patient to the left lateral position and listen with the bell Aortic regurgitation  Normal s1 normal s2  No added sounds  No extra heart sounds Early diastolic murmur beast heard in the tricuspid area whistling in character No radiation Grade 3 Ddx: AR (increases with leaning forward) PR (doesn’t increase with leaning forward) TS (increases with inspiration) MS (increases with turning the patient to the left lateral position) Maneuver: ask the patient to lean forward

Cardio check list: 1. Normal/muffled S1 2. Normal/muffled S2 3. No S3 , S4 4. No ejection clicks or opening snaps 5. No friction rubs 6. Murmur (if present : a. Ejection systolic or pansystolic b. At which area? (Aortic, pulmonary, mitral or tricuspid) c. Radiation to axilla/carotid d. Grading e. Extra Maneuvers (lean patient forward, lean patient to left side, exhale & hold) 7. Diagnosis ? Or DDx
