رجاءا مو بس تسوون الشغلات الروتينيه و هي تدزونه GUE
و بعدين تنطونه Voltaren و amikacin.
او كلش كلش، تسويله rebound tenderness و تدزه US من بره.
In such age, you have to exclude testicular torsion.
و دائما اغلب البيشنتيه ينطوك هستري مال previous attacks و لكن الالم كان dull pain، بس هسه severe pain.
لازم تسويله :
A. Cremasteric reflex.
B. Prehn's sign: releif of pain when the testicle is elevated.
In testicular torsion, the pain isnot elevated 👉👉 the pain isnot releived (negative Prehn's sign)
حريمة تفكر بالـ renal stone or UTI و انت ما مستثني الـ Testicular torsion.
البارحة اجه شاب ١٩ سنة كان delay dx و كان عنده
Blue dot sign (sign of gangrenous testicle)
و للأسف انتهى بـ orchectomy.

احنه الي عدنا هو 20% و يجي 50g/250 cc يعني كل 5g موجودات بـ 25 سيسي
و اكو أشكال أخرى اله
ببساطة، ننطي الدوز كالاتي
0.25-0.5 g/kg within 30 min
يعني الي وزنة 70 كيلو، لازم ننطي 35g، يعني تقريبا
7*25 = 175 cc
لان گلنا كل 5g موجودات بـ 25cc
و الـ 35g راح يكونن موجودات بـ 175cc
تمشيه خلال 15-30 دقيقه

شاب عمرة 37 سنة عنده hypertension
و كان ماشي ع كذا علاج Bisoprolol (concor) و lisinopril و املوديبين
بالفترة الاخيرة، قطع العلاجات كلها (مليت من العلاجات دكتور)
اجه اليوم بـ chest pain، و إلي و لله الحمد كان unstable angina.
بالنسبه إلي توقعت السبب هو
Sudden withdrawal of beta blockers can induced anginal attack
مريض ماشي ع كونكر فترة طويلة، و ال beta receptors كلهن مغلوقات
و فجأة، قطع العلاج، فراح يرجع يشتغل الهارت بسرعه و بصورة مفاجئة، و الي ممكن تسوي انجاينا و حتى MI.
بهجي حالات، لازم تنطي بيتا بلوكر قبل كلشي بالطوارىء، يعني اهمية البيت بلوكر بهجي حالة هي مثل اذا مو اكثر من الاسبرين
