📌 التنبيب الرغامي السريع (RSI - Rapid Sequence Intubation)
🫁 التنبيب الرغامي السريع (RSI)
التنبيب الرغامي السريع هو إجراء يُستخدم لتأمين مجرى الهواء بسرعة وأمان لدى المرضى المعرضين لخطر فشل التنفس أو فقدان حماية مجرى الهواء.
① دواعي التنبيب (Indications)
يُجرى التنبيب عند وجود أحد الحالات التالية:
🔹 عدم القدرة على الحفاظ على مجرى هوائي مفتوح.
🔹 انخفاض مستوى الوعي (GCS ≤ 8).
🔹 فشل أكسجة الدم
(PaO₂/FiO₂ < 250).
🔹 اضطراب معدل التنفس:
أقل من 10 مرات/دقيقة.
أكثر من 30 مرة/دقيقة.
🔹 عدم الاستقرار الديناميكي الدموي (Hemodynamic Instability).
② التحضير (Preparation)
قبل البدء يجب تجهيز:
أجهزة المراقبة:
✅ جهاز مراقبة القلب (ECG).
✅ مقياس الأوكسجين النبضي (Pulse Oximeter).
✅ جهاز قياس ضغط الدم.
✅ خط وريدي (IV Access).
معدات مجرى الهواء:
✅ مصدر أوكسجين.
✅ منظار الحنجرة (Laryngoscope).
✅ أنابيب التنبيب الرغامي.
✅ جهاز الشفط (Suction).
✅ تجهيز أدوية الإنعاش والطوارئ.
⚠️ التحضير الجيد هو أهم خطوة في RSI، ومعظم المضاعفات تحدث بسبب سوء التحضير.
③ الضغط الحلقي وتثبيت الرقبة
الضغط الحلقي (Cricoid Pressure)
يُطبق على غضروف الحلقي بهدف تقليل خطر الارتجاع المعدي والاستنشاق الرئوي.
تثبيت العمود الرقبي (C-Spine Stabilization)
يُستخدم عند الاشتباه بإصابة العمود الفقري العنقي للمحافظة على وضعية الرقبة الطبيعية.
🔸 يُستخدم بشكل انتقائي في الممارسة الحديثة.
④ إعطاء أدوية التحريض والشلل العضلي
أولاً: دواء التحريض (Induction Agent)
💉 Etomidate
الجرعة: 20 mg IV
المميزات:
✔️ بداية التأثير خلال 60 ثانية تقريباً.
✔️ مدة التأثير 3-5 دقائق.
✔️ تأثير محدود على الدورة الدموية.
✔️ مناسب لمرضى الصدمات والإصابات.
ثانياً: المرخي العضلي (Neuromuscular Blocker)
💉 Succinylcholine
الجرعة: 1.5 mg/kg IV
بداية التأثير:
⏱️ 20-50 ثانية.
مدة التأثير:
⏱️ 4-6 دقائق.
موانع الاستعمال:
❌ فرط بوتاسيوم الدم.
❌ إصابات العين النافذة.
❌ الحروق الشديدة.
❌ إصابات السحق.
❌ انحلال العضلات.
❌ تاريخ فرط الحرارة الخبيث.
⑤ إصابات الرأس المغلقة
💉 Lidocaine 100 mg IV
كان يُستخدم لتقليل الاستجابة الودية أثناء التنبيب وتقليل ارتفاع الضغط داخل الجمجمة (ICP).
⚠️ حالياً لا يُنصح باستخدامه بشكل روتيني.
التركيز يكون على:
✔️ التهدئة الكافية.
✔️ التنبيب السلس.
✔️ تجنب نقص الأكسجة.
✔️ تجنب انخفاض الضغط.
⑥ إجراء التنبيب (Intubation)
خطوات التنبيب الفموي الرغامي:
1️⃣ فتح الفم.
2️⃣ إدخال منظار الحنجرة.
3️⃣ رؤية الحبال الصوتية.
4️⃣ إدخال الأنبوب بين الحبال الصوتية.
5️⃣ نفخ الكفة (Cuff).
6️⃣ تثبيت الأنبوب.
⑦ التأكد من مكان الأنبوب
سريرياً:
✅ ارتفاع الصدر بشكل متناظر.
✅ سماع أصوات التنفس في الجانبين.
✅ عدم سماع أصوات بالمعدة.
بالمراقبة:
✅ Capnography المستمر.
⭐ يعتبر ETCO₂ المعيار الذهبي لتأكيد وجود الأنبوب داخل الرغامى.
بالتصوير:
✅ صورة أشعة صدر بعد استقرار المريض.
⑧ التهدئة بعد التنبيب
المرخيات العضلية
💉 Vecuronium
0.1 mg/kg IV
الهدف:
✔️ شلل عضلي طويل الأمد.
✔️ منع السعال وعدم التزامن مع جهاز التنفس.
المهدئات
💉 Diazepam
5-10 mg IV
الهدف:
✔️ التهدئة.
✔️ إزالة القلق.
✔️ فقدان الذاكرة المؤقت.
المسكنات
💉 Fentanyl
200 mcg IV
الهدف:
✔️ تسكين الألم.
✔️ تقليل الاستجابة الودية.
الممارسة الحديثة في العناية المركزة
🔹 التهدئة:
Propofol / Midazolam / Dexmedetomidine
🔹 تسكين الألم:
Fentanyl infusion
🔹 الشلل العضلي:
يُحجز للحالات الخاصة مثل:
ARDS الشديد.
عدم التزامن الشديد مع جهاز التنفس.
Status Asthmaticus.
⑨ الإعدادات الأولية لجهاز التنفس الصناعي
FiO₂
✅ 100% في البداية.
ثم يُخفض تدريجياً للحفاظ على:
SpO₂ > 92%
VT (الحجم الجاري)
✅ 5-7 ml/kg
مثال:
مريض وزنه 70 كغم
VT = 350 - 490 ml
النمط (Mode)
✅ SIMV
يوفر:
أنفاس إجبارية.
ويسمح بالتنفس التلقائي.
الإعدادات المبدئية
🔹 RR = 12 / دقيقة
🔹 PEEP = 5 cmH₂O
🔹 Pressure Support = 0
ثم تُعدل الإعدادات حسب:
✔️ غازات الدم (ABG).
✔️ ETCO₂.
✔️ الحالة السريرية للمريض.
🔹 ملخص RSI السريع
1️⃣ تقييم الحاجة للتنبيب.
2️⃣ تجهيز المراقبة والمعدات.
3️⃣ إعطاء الأوكسجين وتثبيت الرقبة عند الحاجة.

هو استخدام أدوية خاصة لمنع الإحساس بالألم أثناء العمليات أو الإجراءات الطبية، وقد يجعل المريض ينام أو يبقى مستيقظًا حسب نوع التخدير. ويهدف إلى توفير الراحة للمريض، ومنع الألم، والحفاظ على استقرار العلامات الحيوية أثناء العملية.
أنواع التخدير:
أولاً .. التخدير العام (General Anesthesia)
يفقد المريض الوعي بالكامل ولا يشعر بالألم.
يُستخدم في العمليات الكبرى.
يحتاج إلى مراقبة مستمرة للتنفس والدورة الدموية.
ثانياً.. التخدير الموضعي (Local Anesthesia)
يُخدَّر جزء صغير ومحدد من الجسم فقط.
يبقى المريض مستيقظًا.
يُستخدم في الإجراءات البسيطة مثل خياطة الجروح أو علاج الأسنان.
ثالثاً.. التخدير الناحي أو الإقليمي (Regional Anesthesia)
يُخدَّر جزء كبير من الجسم مثل الذراع أو الساق أو النصف السفلي من الجسم.
يبقى المريض مستيقظًا أو قد يُعطى مهدئًا.
من أنواع التخدير الناحي او الاقليمي
: التخدير الشوكي (Spinal) والتخدير فوق الجافية (Epidural).
رابعاً.. التخدير مع التهدئة (Sedation)
تُعطى أدوية تساعد على الاسترخاء وتقليل القلق، مع بقاء المريض واعيًا جزئيًا أو نائمًا نومًا خفيفًا.
يُستخدم في بعض الفحوصات والإجراءات القصيرة مثل المنظار.
أساسيات التخدير (Basics of Anesthesia)
هي المبادئ التي يعتمد عليها طبيب أو تقني التخدير لضمان سلامة المريض قبل وأثناء وبعد العملية.
1. تقييم المريض قبل التخدير (Preoperative Assessment)
أخذ التاريخ المرضي والجراحي.
معرفة الحساسية للأدوية.
مراجعة الأمراض المزمنة والأدوية المستخدمة.
تقييم مجرى الهواء (Airway Assessment).
التأكد من صيام المريض وإجراء الفحوصات اللازمة.
2.اختيار نوع التخدير المستخدم اعتماداً على نوع ومده العمليه وتاريخ المريض ورغبته.
3. تحضير المريض والأجهزة
فحص جهاز التخدير.
التأكد من توفر الأكسجين والأدوية.
تجهيز معدات مجرى الهواء (اللارنغوسكوب، الأنبوب الرغامي، الشفاط).
تركيب قنية وريدية (IV Cannula).
4. مراقبة العلامات الحيوية (Monitoring)
العلامات الحيوية (Vital Signs) هي قياسات أساسية تُستخدم لتقييم الحالة الصحية للمريض، وتُظهر كفاءة عمل القلب والرئتين والدورة الدموية والجسم بشكل عام.
تشمل العلامات الحيويه :
Temperature: 36.5–37.5°C🌡️
Pulse: 60–100 bpm🫀
Respiratory Rate: 12–20/min🫁
Blood Pressure: 120/80 mmHg🩸
SpO₂: 95–100%💨
أهميتها في التخدير:
تقييم حالة المريض قبل التخدير.
مراقبة استقرار المريض أثناء العملية.
الكشف المبكر عن أي مضاعفات مثل انخفاض الضغط، نقص الأكسجين، أو اضطرابات نبض القلب.
مساعدة طبيب التخدير على التدخل السريع للحفاظ على سلامة المريض.
5. إعطاء التخدير
إعطاء أدوية التحريض.
تأمين مجرى الهواء.
المحافظة على عمق التخدير المناسب طوال العملية.
6. المحافظة على وظائف الجسم
ضمان وصول الأكسجين بشكل كافٍ.
الحفاظ على التهوية السليمة.
المحافظة على ضغط الدم والنبض.
إعطاء السوائل والأدوية حسب الحاجة.
7. الإفاقة والرعاية بعد التخدير
إيقاف أدوية التخدير تدريجيًا.
متابعة استعادة الوعي والتنفس.
مراقبة الألم والغثيان والعلامات الحيوية.
نقل المريض إلى غرفة الإفاقة (PACU) بعد استقرار حالته.

🔹حدوث حساسية تجاه بعض أدوية التخدير (نادرة).
🔹مضاعفات خطيرة ((مثل توقف التنفس أو القلب، لكنها نادرة جدًا وتقل احتمالاتها مع التقييم الجيد للمريض والمراقبة المستمرة))
