A 28-year-old man is admitted after a high-speed motor vehicle collision. He is intubated in the emergency department for airway protection. During neurological assessment, he does not open his eyes spontaneously but opens them when pressure is applied to the supraorbital notch. When painful stimulus is applied to the nail bed, he reaches across the midline and attempts to remove the examiner’s hand. Verbal assessment cannot be performed.
