🍁A 4-year-old boy presented with fever and bloody diarrhea, he developed pallor , lethargy and decrease urine output in the past 24 hr. His hematocrit 28% and platelets count of 60,000/cmm. He had protein and blood in the urine
🔻Which the diagnoses is MOST likely to explain these symptoms?
⚡التشخيصات التفريقية :
- gangrene or infected wet gangrene
- necrotizing fasciitis
- advanced Cellulitis
- diabetic foot
نخاف في هذه الحالات من الـ
sepsis and septecemia
وهذا يسبب
septic shock.
📌- مرضى السكر الذي عندهم مشاكل في الكلى ( renal impairment) ويستخدموا انسولين لازم نعدل لهم جرعة الانسولين وننقصها !
ليش؟
🔅لأن الأنسولين يحصله excretion عن طريق الكلى، فلما المريض يأخذ انسولين والكلى حقه تاعبه فهذا يخلي الانسولين يشتغل اطول بالجسم وربما يدخل المريض بـ hypoglycemia لذلك لازم نعدل لهم الجرع وننقصها.
📌- وكذلك الشخص الذي يستخدم heparin وعنده renal impairment لازم ننزل الجرعة والفكرة نفس فكرة الانسولين.🔅
🍂* إذا دوائين subcutaneous لازم نعدل جرعهم وننقصها في حال كان الشخص عنده renal impairment:
♡- insulin
♡- heparin.
⚡Anti-hypertensive drugs⚡
ABCD
A… ACEI
B… Beta blockers
C… calcium channel blockers
D… diuretics
Associated cases with HTN :
HTN+HF = A+D
HTN + IHD = A+B
HTN + Asthma = C
HTN+ pregnancy = B ( labetalol )
HTN + DM = A or C
