A 53 years old male patient with significant past medical history of ischemic heart disease came to ER with history of sever shortness of breath started suddenly. Other past medical history is unremarkable. Upon examination, the patient is conscious, oriented and distress with respiratory rate of: 31, pulse is 112, and SpO2 of: 88. Chest X-ray is unremarkable. ECG showed 'Below'. Cr Clearance 27 . Chest CT with contrast confirm presence of pulmonary Embolism, normal Patient past history ihd, c/o sob not very severe goes with embolism. His bmi is 41 What do you give him A- Ufh 5000 bid B- Ufh 7500 g8 hours C- Enaroxpan 40 bid D- Fondaparinux The answer is B ✅ Because this patient has renal failure with CrCl 27 👍🏻.
