Lec 4 Anatomy ( Embryology of Reproductive system) 🌟Derivatives in male ▪︎Mesoderm of genital ridge---> Interstetial cells of leydig ▪︎Colemic epithelium--->primitive sex cord--->Seminferous tubules + rete testis--->  connect wz each other ▪︎primordial germ cells( طالعة من epiblast= Mesoderm)---> spermatogonia ⚠️كده Seminferous tubules فيها sertoli cells جاية من coelmic epithelium + spermatogonia جاية من ال primordial germ cells ▪︎Mesonephric tubules---> efferent ductules Mesonephric duct---> - epididymis -ductus deferens - caudal bud develops to seminal vesicle - Ejaculatoty duct   ▪︎prostate has 2 origins 1- glandular tissue of prostate---> endoderm of prostatic urethra 2- stroma & smooth muscles ---> surrounding mesenchyme ▪︎Bulbourethral gland---> Bud of Spongy urethra عشان كد بتفتح هنا رغم إنها على جانبي ال membranous urethra ▪️genital tubercle---> elongation--->phallus---> penis ▪️urethral folds---> urethral grooves ⚠️( doesn't reach the end), ectodermal ingrowth --->external  urethral meatus ▪️genital swelling---> scrotal swelling---> scrotum  ▪️Remnants of -Mesonephric duct---> appendix epididymis crainally & paradidymis caudally -paramesonephric duct ---> appendix of testis & prostatic utricle 🌟Derivatives in female ▪️Primitive sex cords---> 1ry medullary cords ---> ovarian medulla( C.T) of ovary ▪️Coelomic epithelium ---> Cortical cords----> Follicular cells which surround Oogonia---> together are called Primordial follicle ▪️Gubernaculum---> Round ligament of avary& round ligament of uterus ▪️Paramesonephric duct -Cranial and middle part ---> uterine tube - Caudal part---> uterine canal ---> Uterus + cervix + upper part of vagina ⚠️ Only Endometrium of uterus is derived from uterine canal Myometrium & parametrium are derived from surrounding mesenchyme ⚠️Vagina has dual origin - upper part is derived from uterine canal( Mesoderm) - lower part is derived from urogenital sinus ( endoderm) ▪️genital tubercle---> clitoris ▪️urethral folds---> libia minora ▪️genital swelling---> libia majora ▪️urogenital groove---> vestibule ▪️Remnants of - Mesonephric duct ( lower part) ---> Gartner's ducts on both sides of vagina - Mesonephric tubules --->epoophroron and paroophoron ☆☆☆☆☆☆☆☆ Note that 🌟 primordial germ cells has an inductive role 🌟uterus didelphys is complete duplication of uterus and cervix ومتحدوش بينما septate uterus اتحدوا ولسه موجود septum بينهم 🌟unicornuate uterus اتكون من paramesonephric duct على ناحية واحدة فبقى one horn 🌟Rudimentary uterus ناحية كملت تكوينها والتانية لا 🌟Arcuate uterus ---> فيه depressed fundus 🌟Hydrocele of testes ---> due to failure of obliteration of processus vaginalis 🌟epispadias may be associated with Ectopia vesica 🔻باقي ال anomalies مكتوب أسبابها واللي اتقال بالنص فالكتاب #EnRe_Anatomy #EnRe_N

🚨Lec 6 physio أهم النقاط في المحاضرة من دكتور زميلنا بارك الله فيها. 🌟FSH act on sertoli cells in seminiferous tubules 🌟LH act on leydig cells in interstitial spaces to secrete testosterone 🌟After 80 ys, spermatogensis declines by 80% 🌟Spermiogenesis = conformational change of spermatid -Testosterone acts on deviding cell until spermatid phase, then FSH stimulates spermiogenesis 🌟testosterone is converted to estrogen in sertoli cells by aromatase enzyme under stimulation of FSH 🌟Inhibin has Inhibitory effect on secretion of FSH + GnRH (long loop + long-long loop) While Estrogen has Inhibitory effect on FSH only (long loop) 🌟-Important vitamins for spermatogenesis -Vit A + Vit C are antioxidant -vit B is antioxidant & catalyzing factor - Vit A inhibits keratinization & atrophy of tubular epithelium 🌟Chemotherapy has toxic effect on deviding cells, besides irradiation 🌟Mumps passes blood- testis barrier--> azoospermia 🌟Tubular fluid = high levels of testosterone + fructose 🌟in epididymis: sperms acquire the capability of motility ,fertility & impaction of PPs and carbohydrates on head to decrease capacitation in male genital tract 🌟fluid secreted from seminal vesicle is clear, mucoid & alkaline While prostate fluid is thin, milky( gives the colour of semen) & alkaline 🌟during ejaculation, central part of prostate closes urethra & peripheral parts allows ejaculated semen to pass to it 🌟After ejaculation within few seconds: 1- clotting enzymes from prostate act on fibrinogen from seminal vesicles ---> seminal clot ( prevents splitting down of semen from female genital tract) 2- primitive liquefaction of seminal clot by PSA 3- complete liquefaction by fibrinolysin ⚠️Dihydrotestesterone is the active form on 2ry sex organs, while testosterone itself is active on testis 🌟Pulsatile secretion of GnRH causes activation of spermatogenesis ( by LH secretion) & continuous secretion causes its inhibition

🌟Lec 4 physio part 2 جزء ال glucocorticoids من دكتور زميلنا بارك الله فيها. 🌟Metabolic effect of glucocorticoids on: - carbohydrates = diabetogenic - protein in extra hepatic tissue= catabolic -protein in hepatic tissue=anabolic ( بيزود تصنيع كل الplasma proteins عدا ال gamma globulins(ABs) ) -fat= catabolic + ketogenic if ⬇️insulin 🌟effect on blood cells -⬆️RBCs, Platelets -WBCs: ⬆️common cells : neutrophils + Macrophages only ⬇️T-lymphocytes (main effect in suppresion of immune system)--->⬇️cellular immunity ⬇️B-lympocytes maturation---> ⬇️ABs production ⬇️eosinphils ⬇️Basophils 🌟Anti-inflammatory effect 1-Stabilization of mast cell membrane + lysosomal membrane--->❌rapture of lysosomes --->❌cell destruction--->❌liberation of kallikrein + ❌liberation of bradykinin---> ❌ increase in permeability--->❌triad of inflammation( redness+hotness+ edema) 2- ⬇️liberation of kallikrein + bradykinin + inflammatorymediators---> ⬇️chemical messengers ---> ⬇️migration of WBCs ---> ⬇️Phagocytosis, tissue destruction 🌟Antiallergic effect by Stabilization of mast cells ⬇️histamine release 🌟glucocorticoids have a mild mineralocorticoid activity 🌟glucocorticoids---> hyperglycemia , ⬆️Amino Acids--->⬇️GH 🌟glucocorticoids promotes production of surfactants from Type ll alveolar epithelium 🌟Control of glucocorticoids -ACTH( Mainly) -cortisol level: -ve feedback effects on Adrenal gland, pituitary, Hypothalamus -stress-->⬆️CRH--->⬆️ACTH -Diurnal rhythm 🌟Cushing syndrome : Pt is liable to - pathological fracture: due to Osteoporosis due to collagen catabolism in bone matrix - fulminiant infection due to suppression of immunity - Thrombi formation: due to ⬆️viscosity due to⬆️RBCS -HTN: due to mineralocorticoid activity -Purple stria: due to atrophy of skin + deposition of fat---> strech ---> rapture of capillaries 🌟 Adrenal Androgens لهم assistant action to main gonades Estrogen in females--> pubic & axillary hair ⬆️Androgens in male before puberty---> precocious puberty + short-stature man ( early closure of epiphyseal plate ) + باقي ال syndrome زي الكتاب 🌟Addison disease -Due to Autoimmune disease or PARTIAL destruction of Adrenal gland -pt has Manifestations of definition of mineralocorticoid, glucocorticoids, Adrenal androgens 🌟Addisonian crisis - caused by ⬇️ cortisol level -شبه ال gastroenteritis بس مع postural Hypotension - بنعالجها ب ringer, NaHco3, glucose supply #EnRe_N #EnRe_physio

🌟Lec 5 pharma -Adrenal gland أهم النقاط من دكتور زميلنا بارك الله فيها. 🔻preparations table مهم جدًا ولازم تعرف اللي بيميز كل واحد فيهم -cortisol--> normal(endogenous) وتأثيره ك gluco& mineralo متساويين -cortisone--> synthetic( exogenous) وتأثيره أقل من ال cortisol -fludrocortisone--> تأثيره ك mineralocorticoid عالي جدًا فعشان كده بنستخدمه فال Addison disease -Predinsone & prednisolone---> تأثيرهم ك Anti-inflammatory أكتر فبنستخدمهم فال osteoarthritis -Methylpredisolone---> أحسن anti-inflammatory & expensive -Triamcinolone & Betamethasone ---> بنستخدمهم لل fetal lung maturation -Dexamethasone ---> Anti-inflammatory فقط 🌟congenital Adrenal hyperplasia---> androgens Causes Ambigious genitalia---> verilization +Hypotension فبديهم corticosteroids 🌟Immunosuppressive effect of corticosteroids makes them effective in ttt of - Autoimmune disorders -leukemia, ITP, lymphoma, multiple myeloma -after Organ transplantation - nephrotic syndrome( ⬇️Ag-Ab reaction) 🌟Anti-inflammatory effect: -Allergic diseases -Bronchial Asthma( mainly) -osteoarthritis -dermatologic uses -interstitial keratitis, post operative inflammation -chrons, ulcerative colitis 🌟other uses - cerebral edema بجرعة صغيرة -facial palsy -spinal cord injury -septic shock meningitis( ⬇️ DIC) ❌side effects to 2ry withdrawal احذفوا منها pseudotumor cerebri 🌟Principles of corticosteroids levels 🔻مهمة فالعملي -فال infant هنستخدم ال ACTH أحسن -Dose must be :low, devided, for short time, increase in surgery , trauma, infection, gradual cessation ⚠️ لو هنكتب prescription ل nephritic syndrome مثلا إيه ال precautions اللي هنكتبها مع ال corticosteroids؟ 1-Vit D + Ca( ❌Osteoporosis) 2-Myogenic agents (❌Peptic ulcer) 3-diet: ⬆️K + ⬇️salt (Na) +⬇️fat + ⬇️carbohydrates 4- follow up of weight, Blood pressure, Blood glucose weekly يعني باختصار هنحاول نمنع كل ال prolonged duration effects 🔻special dosage forms قالت قراءة 🌟Inhibitors of adrenocortical steroids 🔻هناخد منهم 3 بس، ورقم ال CYP مش مهم ⚡️Ketoconazole -antifungal agent -higher doses inhibit cortisone -used in hypercortisolism ⚡️Etomidate - anesthetic -used IV in hypercotisolism ⚡️Aminoglutethimides -inhibits the enzyme which catalyzes the initial and rate limiting steps in biosynthesis of steroids يعني بيوقف كل ال pathway فاحتمال ال toxicity أكتر؛ عشان كده بطلنا نستخدمه ❌Minelcorticoids& antagonistis محذوفين #EnRe_N #EnRe_pharma
