She requires your specialist intervention for fertility treatment and advice on IVF. Mrs Conway had a pregnancy termination ten years ago. Prior to her marriage, she was on Microgynon 30 for five years. She has no history of sexually transmitted disease or any gynaecological problems. Her BMI is 28. On 15/2/17, she reported having been trying to concieve for 18 months, and she was not on any contraceptive pills. Her husband has a job envolving frequent travel. Abdominal and vaginal examinations were insignificant. A cervical smear was taken and blood tests for hormone levels and ovulation were requested. On 15/3/17, Mr an Mrs Conway presented for a review. However, Mr Conway dectined examination and investigation. The blood tests baseline were normal, smear and ovulation tests were negative. She was reasured about the results and was advised about weight reduction and commenced on folic acid 400 micrograms daily. Mr Conway was advised to have semen analysis done. Mrs Daniela has past history of migraines only, she takes no medications regularly. First abdominal pain episode had 01/05/2026 & subsequently had others same like episodes without vomiting & regurgitation with more intensity. Last night she had more intense abdominal pain with vomiting & took 2 aspirin without any relief. She was indicated laboratory tests & abdominal ultrasound. Today, Mrs Daniela is stable and without abdominal pain. Examen Labs indicate Alkaline phosphatase 120 and abdominal US shows multiple gallstones with normal common bile duct on the basis of above I recommend her surgical procedure, thus I refer her to your practice.
