| 2 | ESSENTIAL MEDICAL SUPPLIES: PAIN RELIEF
This will get controversial in a bit but please read it in the spirit it's intended - survival in case of civil collapse for whatever reason.
ACETAMINOPHEN / PARACETAMOL
Excellent for mild to moderate pain. Often administered via IV in the ER. Caffeine is likely to enhance the effect of the drug. Paracetamol is very useful in treatment of fever. Can be combined with Ibuprofen or other non-steroidal anti-inflammatory drug (NSAID). Widely available. Do not use if you have impaired liver function.
IBUPROFEN
Most widely available NSAID. As mentioned above can be used at the same time as paracetamol. It's anti-inflammatory and beloved by the Royal Marines as 'bufren' to get them through the endless yomping (yomping is what the paras call a pleasant stroll). Can cause stomach problems in high doses or if used long term. Do not take on an empty stomach.
OTHER NSAIDS
These include Diclofenac and Naproxen. A doctor should have no issue with prescribing these for pain as they are not controlled substances. 'Stronger' than ibuprofen. Can be very effective painkillers. Same warnings as for ibuprofen. Available from the pharmacist over the counter in some countries including UK.
OPIATES
There are 'weak' and 'strong' opioids. The strength is measured against 10mg instant release (IR) Morphine equivalent.
Essentially all opiates are converted into morphine by the body.
Weak opioids include codeine, dihydrocodeine and tramadol. Tramadol has a different and novel mechanism of action which is too complicated to outline here.
Strong opioids include morphine, diamorphine and oxycodone - but there are many more.
In my opinion it is pragmatic to have a supply of opioids for use in an emergency when professional medical help is not available.
Again in my opinion the minimum you should have to hand is codeine - 30mg or 60mg tablets. If you have never used opioids before then 60mg codeine will give you substantial pain relief.
If you have or had addiction problems or a history of substance misuse disorder in your family then I suggest you stay well away from opioids. However, the decision is yours.
ASK YOURSELF CAN YOU KEEP IT IN YOUR KIT AND NOT USE IT OR EVEN THINK ABOUT USING IT UNTIL YOU ARE IN SEVERE PAIN? If so, it might be right for you to have it in your kit.
(As an aside, codeine will also stop you coughing.)
I was going to outline how to get a legitimate prescription from your doctor (including for strong opioids) but decided that this is a bad idea.
Instead I will give you two terms: Idiopathic Back Pain.
