We need to change our perceptions of patients and health care professionals about factors contributing to Medication Errors and potential areas for improvement. A wide range of factors perceived as contributing to medication errors was identified. These results provide valuable information that could be used to improve the medication use by applying the concept of systems thinking 💭 We need to address the whole journey from the prescription writing to the dose given to analyze the faults to be managed and to be prevented. Cause-and-effect tool through icebergs model or fishbone root causes analysis. The below causes are just examples represent the perceived factors leading to errors within each category or subcategory: 🔥People factors: - The pharmacist does not review the medication before dispensing it. - Nurse distraction - Not following the 5R’s ( Right dose, right drug, right person...etc ) - Multitask - etc 🔥Management factors: - Nurse sleep deprivation syndrome - Working overtime. - etc 🔥Process factors: - The second check by the nurse was not performed - etc 🔥Equipment factors : - No label scanner - Label not clear - etc 🔥Environment factors : - Round interrupted - Work overload - High noise environment - etc 🔥Communication factors: - Handwritten order eligible - Transcription errors - etc For more information follow us at: https://t.me/SystemsThinkingMastery
