Treatment for: High Cholesterol, High Cholesterol - Familial Heterozygous
Lipfendra (enlicitide) is an oral proprotein convertase subtilisin kexin type 9 (PCSK9) inhibitor to reduce low-density lipoprotein cholesterol (LDL-C) in adults with hypercholesterolemia, including heterozygous familial hypercholesterolemia (HeFH).
✅Lipfendra is indicated as an adjunct
to diet and exercise to reduce LDL-C in adults with hypercholesterolemia, including HeFH.
💹High LDL-C is a major risk factor for atherosclerotic cardiovascular disease (ASCVD), the leading cause of death globally.
⏩Lipfendra is a novel macrocyclic peptide that binds to proprotein convertase subtilisin kexin type 9 (PCSK9) and inhibits the binding of PCSK9 to LDL receptors, increasing the number of receptors available to clear LDL-C and thereby lowering LDL-C levels. It is the first oral PCSK9 inhibitor approved to reduce LDL-C.
FDA approval of Lipfendra was supported by positive clinical results from two pivotal Phase 3 trials in the CORALreef clinical program: CORALreef Lipids (NCT05952856) and CORALreef HeFH (NCT05952869).
✅In CORALreef Lipids, at week 24, Lipfendra reduced LDL-C by 56% versus placebo (60% in a post-hoc analysis), with mean reductions of 54% in non-HDL-C and 50% in ApoB.
In CORALreef HeFH, at week 24, Lipfendra reduced LDL-C by 59% versus placebo, with mean reductions of 52% in non-HDL-C and 48% in ApoB.
✅Lipfendra is administered as one 20 mg tablet taken by mouth each morning on an empty stomach with water, black coffee, or plain tea. The tablet is swallowed whole, and patients should wait at least 30 minutes before eating or drinking anything other than water, black coffee, or plain tea.
🆘Common adverse reactions:
In CORALreef Lipids, the frequencies of adverse reactions in adults with hypercholesterolemia were similar between those treated with Lipfendra and those receiving placebo.
In CORALreef HeFH, the most common adverse reactions in adults with HeFH that occurred at higher frequencies compared to placebo were diarrhea (Lipfendra 7%, placebo 2%) and dizziness (Lipfendra 9%, placebo 4%).
In both trials, similar proportions of Lipfendra-treated patients and placebo-treated patients discontinued treatment because of an adverse reaction.
Lipfendra is continuing to be evaluated in the large cardiovascular outcomes trial CORALreef Outcomes (NCT06008756), which has completed enrollment with over 14,500 participants.
