المحاضرة باختصار: بدأ ب case عن بنت جاية ب CD و fistula في ال rt iliac fossa و انها كانت محتاجة جراحة بس لازم قبل الجراحة نعمل control لل inflam عشان نحد من ال fistula formation و لأنها جاية في aggressive stage هنديها علطول ال biological therapy اتكلم عن واحد منهم اسمه Stelara دة بنستخدمه في حالات moderately to severely active crohn's dis و باقي الأمثلة زي ال RA اتكلم عن مكان ال affection في كل واحد CD & UC و ال presentation و ان اشهر systemic presentation معاهم هي ال rheumatological ( AS ( و كمان فيه ال primary sclerosing cholangitis كتاب القسم كاتب كمان autoimmune hepatitis بس الدكتور قال هي مع PSC دكتور مدحت شرح كدة بردة اتكلم عن Hidradenitis Suppurativa HS A chronic relapsing fistulizing skin disorder affecting apocrine glands ممكن عيان ييجي ب peri-anal abscess ال DDx هنا - abscess in an immunocompromised patient - HS - CD بعمل colonoscopy عشان افرق و ممكن معرفش افرق بس ال biological therapy بستخدمه كمان مع HS  في ال ttt دة اللي قاله : ممكن نفقة الدولة تتأخر علي ما نقدر نعالج ب Biological therpay فاللي هعمله - Traditional therapy for any auto-immune dis - steroids for induction of remission Then tapering and maintenance by azathioprine or cyclosporine of no response to azathioprine ( steroid-sparing immunosuppressive drugs ) ال azathioprine بياخد من شهر ل تلاتة علي ما يبدأ الفترة دي بدي فيها ستيرويد دوا تاني نقدر نستخدمه في ال maintenance هو masalamine أو pentasa دة local anti-inflammatory بنديه ب high doses بطريقة تملي ال colon و terminal ileum فمش هينفع مع حالات ال small intestine affection بيتاخد علي هيئة oral sachets أو نديه suppositories ( دي خصوصًا مع حالات ال ulcerative proctitis )أو enema If no response: biological therapy Surgical therapy in UC : In case of resistance to medical TTT - colectomy with ileo-rectal anastomosis و دة الدكتور بيحبه ب يسيب ال rectum و لو حصل affection يعتمد علي local TTT - the curative TTT is pan-proctocolectomy * IBD is pre-cancerous ( follow-up by colonoscopy yearly ) and cancer is indication for colectomy.

Asthma >> The cornerstone in ttt of asthma patients>> inhaled corticosteroids and the most important during the acute severe asthma >> bronchodilator. In acute severe asthma Silent chest means that the bronchospasm is so severe that you can't hear any air flow.
