-------------------------------------------------------

❤️ If pregnancy occurs, urgent counselling by a specialist Pregnancy Heart Team is essential; termination should be discussed where appropriate. ━━━━━━━━━━━━━━━━━━
💜 6. VALVULAR HEART DISEASE
🚨 Severe mitral stenosis and severe aortic stenosis can make pregnancy extremely hazardous and may be considered contraindications depending on severity, symptoms and haemodynamic status. 🩺 Balloon valvuloplasty or transcatheter intervention may be considered in carefully selected refractory cases. 💗 Bioprosthetic valves may be preferred for women planning pregnancy (Class I, Level B). ⚠️ Mechanical valves carry substantial maternal and fetal risks; ROPAC data cited in the supplied summary report approximately 54% event-free pregnancy with live birth. 💊 Mechanical valves require meticulous, individualised anticoagulation planning. ━━━━━━━━━━━━━━━━━━
🔵 7. ANTICOAGULATION IN PREGNANCY — MECHANICAL VALVES
💊 First trimester: The supplied 2025 ESC/EACTS summary describes a dose-based approach: warfarin at ≤5 mg/day may favour continued VKA treatment; higher doses may prompt consideration of dose-adjusted, twice-daily LMWH with anti-Xa monitoring. 📅 Around week 36: Plan transition to UFH/LMWH before delivery under specialist supervision. 🧪 Anti-Xa monitoring: Individualise targets for mechanical valves; the supplied ESC pregnancy summary cites a peak target around 1.0–1.2 IU/mL. 💉 Enoxaparin dosing must be adjusted to anti-Xa results rather than relying on a fixed mg/kg dose alone.
🚫 DOACs are contraindicated for mechanical heart valves during pregnancy.
⚠️ Anticoagulation changes can cause catastrophic valve thrombosis or bleeding. Management must be coordinated by a Pregnancy Heart Team and anticoagulation specialists. ━━━━━━━━━━━━━━━━━━
🩷 8. HYPERTENSION IN PREGNANCY
🎯 Blood pressure target: Below 140/90 mmHg, according to the supplied guideline summary. 💊 Acute severe hypertension may require IV labetalol, urapidil or nicardipine; oral nifedipine or methyldopa may also be used according to the clinical setting and local guidance. 🚫 Avoid ACE inhibitors, ARBs and ARNIs during pregnancy. ━━━━━━━━━━━━━━━━━━
❤️ 9. ACQUIRED HEART DISEASE
🫀 Acute coronary syndrome: Do not delay necessary assessment and treatment because of pregnancy; individualise management with a multidisciplinary team. 💊 DAPT: Tailor antiplatelet selection and duration to ischaemic and bleeding risks. 🧪 Statins: May be considered in selected women with established ASCVD following specialist risk–benefit assessment. 🚑 Cardiac arrest: Manual left uterine displacement during CPR is recommended when the uterus is large enough to cause aortocaval compression, commonly around 20 weeks or later. 🩺 Heart transplantation: Pregnancy is generally discouraged during the first year after transplantation; immunosuppression and graft function require close monitoring. ━━━━━━━━━━━━━━━━━━
🌺 10. ADVERSE PREGNANCY OUTCOMES (APOs)
⚠️ Preeclampsia, preterm birth and intrauterine growth restriction are associated with increased future cardiovascular risk. ❤️ Women with these pregnancy complications should receive cardiovascular risk assessment, counselling and appropriate long-term follow-up. ━━━━━━━━━━━━━━━━━━
🌷 11. CONTRACEPTION & ASSISTED REPRODUCTION
💗 Options often suitable for women with cardiovascular disease: levonorgestrel IUD, copper IUD and progestin implant, depending on individual circumstances. 🚫 Combined oral contraceptives may be unsuitable in high-risk groups, including some women with thrombosis risk, Fontan circulation or mechanical valves. ✅ Emergency contraception is generally available to women with cardiovascular disease; select the method according to clinical circumstances. 👶 Assisted reproduction: Single-embryo transfer is recommended to reduce multiple-pregnancy risk (Class I, Level C). ━━━━━━━━━━━━━━━━

🫀🌸 CARDIOVASCULAR DISEASE & PREGNANCY 🌈 KEY GUIDELINES & PRACTICAL CLINICAL PEARLS | 2025–2026
━━━━━━━━━━━━━━━━━━
🔴 1. ARRHYTHMIAS
❤️ Long QT Syndrome (LQTS)
💊 β-blockers (nadolol or propranolol) are recommended during pregnancy and lactation (Class I, Level B); continue the pre-pregnancy dose postpartum.
➡️ LQT2 carries the highest postpartum risk, extending up to 12 months — maintain treatment and vigilance throughout this period.
🍼 Lactation caution: Nadolol has greater breast-milk excretion (relative infant dose approximately 4–7%); monitor infants for bradycardia, particularly with high maternal doses. Consider any switch to propranolol before pregnancy rather than changing treatment during pregnancy or postpartum. 💗 Consider left cardiac sympathetic denervation before pregnancy if arrhythmias remain uncontrolled.
🌸 Catecholaminergic Polymorphic VT (CPVT) 💊 β-blockers (nadolol/propranolol), with flecainide added if arrhythmic events occur (Class I, Level C).
➡️ Continue established treatment during pregnancy and postpartum; consider sympathetic denervation in refractory cases.
💙 Brugada Syndrome
💊 Quinidine may be considered if arrhythmic events occur during pregnancy (Class IIa, Level C).
💜 Short QT Syndrome (SQTS)
💊 Continue quinidine throughout pregnancy and postpartum when indicated (Class IIa, Level C).
🚨 General antiarrhythmic caution Amiodarone is not recommended routinely during pregnancy; reserve it for refractory, life-threatening ventricular arrhythmias, with fetal monitoring for bradycardia and intrauterine growth restriction.
━━━━━━━━━━━━━━━━━━
💗 2. PERIPARTUM CARDIOMYOPATHY (PPCM)
✅ Counsel every woman about recurrence risk, even after ejection fraction (EF) recovery (Class I, Level C).
💊 Consider bromocriptine with prophylactic LMWH in selected patients (Class IIa, Level C). 🧬 Genetic counselling and testing may be considered.
❤️ Treat according to heart-failure guidance and generally continue therapy for at least 12 months after EF recovery, with individualised reassessment. ⚠️ Important clinical nuance: The 2025 SMFM Consult Series #73 advises against β-blockers in acute decompensated LV failure, favouring hydralazine/nitrates and furosemide for acute management; ACE inhibitors, ARBs and ARNIs are reserved for postpartum use. Individualise treatment according to haemodynamic status and specialist advice. ━━━━━━━━━━━━━━━━━━
🟢 3. AORTOPATHIES
🧬 Assess inherited aortic disease, including Marfan syndrome, Loeys–Dietz syndrome, bicuspid aortic valve (BAV), Turner syndrome and vascular Ehlers–Danlos syndrome. 📏 High-risk aortic thresholds highlighted in the supplied summary:
Marfan syndrome: aortic diameter greater than 45 mm
BAV: aortic diameter greater than 50 mm
Turner syndrome: aortic size index (ASI) greater than 25 mm/m²
Vascular EDS: pregnancy may carry extreme risk and may be contraindicated. ❤️ Pre-pregnancy counselling is essential after aortic dissection or surgery.
💊 β-blockers during pregnancy and postpartum are recommended for Marfan syndrome/heritable thoracic aortic disease when indicated (Class I, Level C).
━━━━━━━━━━━━━━━━━━
🟣 4. CONGENITAL HEART DISEASE (CHD)
🔹 Fontan circulation: Pregnancy is strongly discouraged in many patients because of substantial maternal and fetal risks. 🔹 Systemic right ventricle: Following Mustard/Senning repair or in congenitally corrected TGA, pregnancy requires specialist risk assessment and counselling. 🔹 Eisenmenger syndrome: Pregnancy is contraindicated because of extremely high maternal risk. 🔹 Simple repaired ASD/VSD/PDA: Often lower risk when there are no significant residual lesions or complications. ━━━━━━━━━━━━━━━━━━
🟠 5. PULMONARY ARTERIAL HYPERTENSION (PAH)
🚨 Pregnancy is contraindicated in PAH because of potentially life-threatening maternal risk.
