👁️ Proptosis → think of Thyroid Eye Disease (TED) until proven otherwise, especially if bilateral + axial proptosis.
⇨Bilateral axial proptosis → TED is a key diagnosis to rule out.

• Cobra/Krait: Neurotoxic → ptosis → respiratory paralysis. • Viper: Hemotoxic → bleeding, coagulopathy, local necrosis. • Sea snake: Myotoxic → rhabdomyolysis + myoglobinuria. • Snake-bite marks: Fang marks may be absent → absence does not exclude envenomation. • Arsenic: Rice-water diarrhea + vomiting; chronic → pigmentation, hyperkeratosis, Mees lines. • Mercury: Tremor + erethism + gingivitis. • Lead: Colic + constipation + Burtonian line + wrist drop. • Yellow phosphorus: Severe hepatic injury; chronic → phossy jaw. • Cantharides: GI/urinary irritation + hematuria. • 1st degree burns: Erythema + pain, epidermis only, no blister. • 2nd degree burns: Blisters + severe pain, partial-thickness burn. • 3rd degree burns: Full-thickness destruction → dry, leathery, usually painless. • 4th degree burns: Extends to muscle/bone → charring. • Antemortem burn: Vital reaction + soot in airway/COHb when smoke inhalation occurred. • Postmortem burn: No vital inflammatory reaction. • Pugilistic attitude: Flexion due to heat-induced muscle contraction; not evidence of struggle. • Rule of Nine: Adult burn surface-area estimation; Lund-Browder is more accurate in children. • Heat hematoma: Can mimic an extradural hematoma. • Heat fractures: May mimic traumatic skull fractures.#Forensic_Medicine_Toxicology

● Optic Neuritis | Why is it painful?
الـ Optic nerve موجود داخل muscle cone وقريب ومحاط بـ EOMs والـsurrounding tissues.
عند حركة العين ← بتتحرك العضلات والأنسجة المحيطة ←
traction/movement on the inflamed optic nerve → Pain with eye movement.
🔑 Optic neuritis → Pain, especially with eye movement.

Meningitis VS Meningism/Meningismus.🔹 Meningitis = التهاب حقيقي فِ الـ meninges، غالبًا بسبب infection. 🔹 Meningism = Meningeal signs/symptoms مثل neck stiffness + headache + photophobia ليكن بدون التهاب حقيقي للسحايا، وممكن يحدث فِ: Severe pneumonia, sepsis, intracranial hemorrhage, migraine وغيرها. ← الفكرة إنه فِ Meningismus البيشنت عنده meningeal irritation/signs، بس الـ meninges نفسها مُش ملتهبة بالضرورة.
- Meningitis = inflammation. - Meningism = signs without inflammation.

Heterochromia.= different iris colors. اختلاف لون القزحيتين أو اختلاف اللون داخل القزحية نفسها. • Types: - Complete: كل عين بلون مختلف. - Sectoral/partial: جزء من نفس القزحية بلون مختلف. - Central: اختلاف اللون حول الـ pupil بشكل حلقي. #ophthalmology

• Hordeolum → Painful acute lid lump. • Chalazion → Painless chronic lid lump. • Lagophthalmos → Incomplete closure. • Orbital cellulitis → Proptosis + painful EOM. • Preseptal cellulitis → Lid swelling without orbital signs. • Pterygium → Fibrovascular tissue crosses cornea. • Pinguecula →Yellow lesion, doesn't cross cornea. • Viral conjunctivitis → Watery + preauricular node. • Allergic conjunctivitis → Severe itching. • Bacterial conjunctivitis → Purulent discharge. • Dacryocystitis → Medial canthal swelling. • NLD obstruction → Epiphora. • Medial orbital wall → lamina papyracea → very thin. • Orbital floor fracture → infraorbital nerve numbness + diplopia. • Inferior rectus is commonly involved in thyroid eye disease. • Papilla → allergy. • follicle → viral/chlamydia. • Recurrent chalazion in an older patient → exclude sebaceous carcinoma. • CN VII palsy → inability to close eye → exposure keratopathy. • Acute proptosis + visual loss + high IOP after trauma → orbital compartment syndrome.#ophthalmology
