A 26-year-old man is brought to the Emergency Department after a road traffic accident. On arrival, his GCS is 14/15 (E4 V4 M6). Initial CT brain shows a small temporal contusion without mass effect, and he is admitted for close observation.
One hour later, the nurse notices increasing drowsiness. Repeat examination shows GCS dropped to 12/15 (E3 V3 M6). Blood pressure is stable, oxygen saturation is 98% on room air, and pupils remain equal and reactive. No seizures occurred.
