There were also comminuted and multifragmentary fractures involving the hard palate, ethmoid bone, orbital lamina, and left maxillary sinus. Additionally, metallic foreign bodies were observed within the right eye cavity, along with bone fragments from the orbital floor (Fig. 2). >>Given the condition of the facial bones and the evident respiratory difficulty, the patient was sedated and intubated. Fluid infusion and antibiotic prophylaxis were initiated. Considering the clinical presentation, the patient was urgently taken to the operating room to control the ongoing facial bleeding and perform a temporary tracheostomy. >>A post-intubation arterial blood gas (ABG) analysis, with a fraction of inspired oxygen (FiO2) at 40%, showed the following results: pH 7.213 (normal range: 7.350–7.450), pCO2 58.3 mmHg, pO2 87.4 mmHg (normal range: 75.0–100.0 mmHg), ctHb 8.6 g/dL (normal range: 12.0–18.0 g/dL), FO2Hb 92.5% (normal range: 94.0–97.0%), and cLac 8.3 mmol/L (normal range: 0.3–2.5 mmol/L). >>Five days after the initial admission, the patient’s general condition stabilized and surgical reconstruction of the facial bones was carried out with three-dimensional modeled titanium reconstruction plates and meshes. Soft tissue reconstruction was achieved using local flaps. The extensive lacerated-contused wounds provided access to the damaged right mandible and orbito-malar region, which had experienced significant bone loss. Mandibular reconstruction was performed using a plate, and the orbito-maxillo-malar and nasal regions were reconstructed using appropriately modeled and fixed titanium meshes and screws. Necrotic skin flaps were excised, and skin flaps were created. Sliding flaps were used to fill the cutaneous deficits. >>A postoperative CT scan was performed to confirm the presence of synthesized materials (Fig. 3). The postoperative course was uneventful. The tracheostomy was removed 3 days after the surgery, and oral feeding was gradually reintroduced in the days that followed without oral cavity infection or functional swallowing issues. The patient was discharged from the hospital 10 days after the surgical intervention. >>Follow-up visits in subsequent weeks showed regular healing and gradual stabilization of scar outcomes and functional outcomes. Six months post-surgery, despite satisfactory wound healing, the patient was offered a facial reconstruction procedure using microvascular free flaps to achieve better aesthetic and functional restoration of the face and facial skeleton (Fig. 4).

Rupperswil murder case Story Time Well this happened around 2014/2015 if I remember correctly here in switzerland. I find it especially disturbing, since it was right about 2 villages away from where I live and many people knew the victim personally. Also you never hear a story like this in switzerland. It started with a news article about a house burning down. and I remember a friend in my class telling me its the house of his friend and both their families wer really close for years. a day later, when they managed to put the fire down, they discovered 5 corpses inside. turns out, a guy held them hostage in their own house and would compell the mother to withdraw a lot of money. later the security fotages wer released in which you could see how nervous, and traumatised she looked. He ended up stealing around 10'000 Sfr. Worst part is, It was a whole family, the mother, 3 sons (aged 9, 13 and 17) and the girlfriend of the oldest brother. authopsies later have shown, that, the three sons have been raped multiple times, probably in front of the others eyes. The offender wasn't caught until two years later in the middle of the city with weapons and stuff to torture people. Really horrible story. I remember my friends mom mourning and sobbing for days. edit: I searched the case up and it didn't went down exactly, as I remembered. The patient underwent a complicated operation comprising of debridement of devitalised tissue, soft tissue reconstruction and external fixation of broken bone segments. This 33-year-old man received a face transplant just three weeks after being disfigured in a workplace accident, in what doctors said is the fastest time frame to date for such an operation. Face transplants are complicated and relatively rare procedures that usually require extensive preparation, typically months or years. But this patient's condition was deteriorating so rapidly that a transplant was seen as the only option. He was inured in an April 23 accident at his job at a stone mason's workshop when a machine used to cut stone tore off most of his face and crushed his upper jaw. He received intensive treatment at a hospital that saved his life and eyesight. But an attempt to reattach his own face failed, leaving an area close to the brain exposed to infections. The damage was too extensive for doctors to temporarily seal the exposed areas. He underwent a total face transplant in a 27-hour operation. The surgery reconstructed the area around the eyes, nose, jaw and palate and other facial areas, with the transplant running from above his right eye, under his left eye and around his face to his neck. The donor, a 34-year-old man, was chosen from a national registry of potential donors after his age, gender, blood group and body features were determined to be a good match for the injured man. Six days after the surgery, he was photographed making a thumbs-up gesture from his hospital bed.
