The 58-year-old patient presented with excruciating penile pain, swelling, and a foul, putrid odor emanating from his genitals. He had been using topical ointments with no relief for the past two weeks.
The patient's medical history was complicated, as he suffered from chronic renal disease requiring hemodialysis for over 4 years, hypertension, type 2 diabetes, and had experienced an ischemic stroke 5 years prior. He also had secondary hyperparathyroidism and regularly took various medications, including cinacalcet, isosorbide, NPH insulin, gabapentin, and captopril. Disturbingly, the patient also had a history of smoking and alcohol consumption.

Upon examination, the clinicians were confronted with a gruesome sight. The patient's entire penis was enveloped in a state of wet necrosis, with widespread hyperemia and a revolting, foul-smelling discharge. Attempts at debridement only served to hasten the progression of the necrosis, and within just 5 days, the entire penile shaft had become completely necrotic. The decision was made to perform a total penectomy, and the clinicians then fashioned a perineal urethrostomy to allow for urination.
The patient was treated with a regimen of ceftriaxone and metronidazole antibiotics, and over the course of 5 weeks, he gradually began to improve. The anatomopathological examination revealed the presence of ischemic necrosis, Monckeberg atherosclerosis, and an acute inflammatory infiltrate. The patient is currently being monitored by various clinical treatment teams and is reported to be clinically stable, though the devastating loss of his penis has undoubtedly had a profound impact on his physical and psychological well-being.
