💥Defibrillation 💥 👉is the definitive treatment for immediately life threatening daysrythmia حاله المريض بتاعى بتكون emergency و كمان أهم حاجة أنه not have Pulse زى مريض cardiac arrest محتاج كمية كهربية150-200 و بنعمل CPR أثناء ال shock عكس ال cardio version مش بنعمل معاها ✨🐬✨Indications 👉shockable Rhythms 🐬PVT 🐬VF 🐬cardiac arrest ✨🐚✨contraindication 👉Non shockable 🐚Asystole or Standstill 🐚PEA ✨💝✨أنواع ال Defibrillators 💝 Automated External Defibrillators ( AED) فى حالة pre hospital care مش بيتطلب تدريب على استخدامه أى حد يقدر يستخدمه It analyzes the heart rhythme , Charges 💝semi AEDs شبه AED بس الفرق أنه فيه button ال rescuer بيضغط عليه عشان push the required shock 💝Implantable Cardiovertion Defibrillator النوع ذه يتزرع تحت جلد المريض زى Pacemakers من خلاله نقدر نشوف heart rhythm 💝standard Defibrillators ده اللى احنا بنتكلم فيه دلوقتى وده بيكون حاجة من اثنين mono pbasic , biphasic وده بيكون فى الطوارئ، أو ICU 🛸يلا بينا نعرف كل واحد بيشتغل ازاي 🕸️monphasic🕸️ ال electrical current بتكون في اتجاه واحد وطالما هي هتروح ومش هترجع تانى يبقا الكهربية كبيرة 360 🕸️Biphasic🕸️ ال electrical current ليها اتجاهين لما أعطى الكهربا أول مره أو ببدأ 150-200 وبعد كدا المرات الجايه هتزيد و هتبقا150-300 كل ده عشان أمنع ان يحصل Burn ✨👻✨PROCEDURE 👉Assessment 👻Assess the Cardiac status بمعنى هنشوف ال indication ( PVT , Vf) وأنه معندوش pulse 👉preparation 👻Environment نحافظ علي خصوصية المريض نحضر ال equipment بالنسبة للمريض 👻romove any metallic objects dentures 👻نضع تحت المريض backboard 👉Implementation 👻نبدأ الأول CPR لحد ما الجهاز بتاعى يجهز 👻ال crashcart تكون جنب سرير المريض 👻نضع على ال Paddles كميةصغيره من الجيل عشان ميحصلش حرق 👻كل paddle بيكون مكتوب المكان اللى هتكون فيه 👻إزاى بنضع ال Paddles واحدة عند 👉at the heart's apex just to the left of the nipple in the midaxillary line . التانية 👉 just below the right clavicle to the right of the sternum 💥نحضر جهاز ال defibrillator synchronizer button switched off 👻 نغير فى الشحنه الكهربية على حسب الموصوفة ليا ( ACLS protocol مؤسسة القلب الامريكية) 👻Apply suitable pressure to each paddle against the chest wall ال Paddles الأثنين بيكونوا متساويين فى الضغط اللى على صدر المريض 👻المكان بتاعى يكون خالى من الناس . 👻لو المريض بتاعى متوصل بالأكسجين أفصله 👻وأنا يعطى للمريض shock عينى على cardiac rhythm 👻لو المريض بتاعى لسه shockable أعمله CPR حسب ACLS اننا نبدأ Chest compressions بعد دقيقتين من CPR ولسه برضو Shockable أعطيه shock بالجهاز تانى 👉post care ✨🐬✨احنا كنا في الأول بنعمل كل ده عشان نرجع ال pulse ف هنشوفه 🐬لو ال pulse رجع معايا هنضع المريض side line position وو ننقله لل ICU عشان نعمله post resuscitation Care 🐬لو لسه مرجعش معايا ننقل المريض expected area 🐬نعمل assess للجلد لو حصل حرق

🧚Nasogastric tube insertion : هركز علي الارقام والأوضاع ✨NGT : (8:18) french ✨tommey syringe: (30:60)cc ✨بخدره علي الاقل قبل ما ادخلها ب 5mine ✨ lubricate 6:10 cm to prevent friction ✨عشان تتأكد أنها دخلت في مكانها الصح في أربع طرق 🧚inject 15:20cc of air واسمع بقي المفروض اسمع bubbling noise 🧚جيب سرنجة مقاسها 50ml واسحب محتوي المعده وشوف الph المفروض أقل من 4 لو لقيت وسط قلوي اكبر من 7 يبقي انت في small intestine ولون محتوي المعده اصفر قالب علي اخضر كده 🧚 بعد ما بتخلص بقي سواء lavage, feeding, medication هتعمل اي بقي : 👉Irrigation with 20:30 cc of normal saline ✨Positions 💥عند التركيب : 👉High Fowler position 👉Hed of bed =45° 💥عند الوصول ل nasopharynx: 👉Flex head forward وتخليه يشرب ميه اول ما يوصل للمكان دا 💥لما بتخلص بقي تركيب : 👉semi Fowler position

👉is a medical procedure that restores a normal heart rhythm إجراء طبى عادى إحنا بتعمله عشان نرجع rhythm لحالته الطبيعية المريض الكهربا بتاعته مش منتظمة و بيكون جاى وعارف انه هيعمل أو هياخد shock ف بكده نعرف ان المريض conscious يعنى ال Pulse بتاعه موجود عكس defibrillation ✨🔮✨Indications 🐿️supra ventricular Tachycardia 🐿️Aria fibrillation 🐿️Arial flutter 🐿️VT with pulse ✨🔮✨contraindication = Indication, contraindication of defibrillation ✨🔮✨أنواعه أي 👉Pharmacologic = anti arrythmias drugs بنرجع ال ى rhythme بالأدوية 👉Electrical Cardioversion = shock موضوع حلقتنا النهارده نفس ال Defribrillation بس هنقول الاختلافات 🥰 👻نقيس ال vital sign قبل ال Procedure 👻ان المريض بتاعى يكون صايم على الأقل6 ساعات عشان ميحصلش Vomiting أو aspiration 👻نعمله تحاليل لل K ، ABG، INR ،Ca ،area 👻أعطى للمريض بتاعى قبل ال procedure( seductives) 👻لو المريض بتاعى بياخذ digitalis أوقفه قبلها48 ساعة قبل ال Procedure 👻أثناء تحضير Cardioversion The synchronize button switche on 👈وبس كده دول الاختلافات فيه شوية احتياطات لازم أخدها😩 ✨💖✨المكان بتاعى يكون مناسب خالى من الناس أو أى metalic objects ✨💖✨لو المريض بتاعى متوصل بالأكسجين أفصله ✨💖✨Never use defibrillator with improper grounding or electrical leak socket . م نستخدم defibrillator مش مناسب أو فيه أى تسريب ✨💖✨Keep the defibrillator away from any x - ray , ultrasonic or other electronic instruments . الجهاز يكون بعيد عن أى أجهزه الإشعاعية من الآخر ✨💖✨Excessive amount of conductive gels on the chest wall can cause burns on the chest wall . بنستخدم كمية صغيره من الجيل عشان م حرق وكذالك م نستخدم الكحول ✨💖✨ Malfunction of permanent pacemakers can result from defibrillator pads or paddles near the pacemaker . ✨💖✨Never discharge the defibrillator in air to check its performance . ✨💖✨Do not make friction of DIC paddles . لما نستخدم الجهاز منعملش لل Paddles أى احتكاك ببعضه ✨💖✨ Ensure that all cables and wires in intact daily . أتأكد من ان جميع الكابلات و الاسلاك معايا سليمة ✨💖✨Keep the D / C machine all the time in electrical charge . الجهاز لازم يبقا مشحون معايا علطول
