أكيد مرّت عليكم حالة بالعيادة: أخذنا Local anesthesia بشكل صحيح، والمريض بعده يحس بالألم 😭
خصوصًا إذا السن بيه acute inflammation.
بس شنو السبب؟
🔥 1. انخفاض الـ pH
بالأنسجة الملتهبة يصير الـ pH أكثر حموضة، وهذا يقلل من نسبة الـ Local anesthetic اللي تكون بالشكل القابل لدخول الـ nerve membrane، وبالتالي تقل فعالية التخدير.
🧠 2. الـ inflamed nerve يكون أكثر حساسية
الالتهاب يخلي الـ nociceptors أكثر استجابة، لذلك المريض ممكن يبقى يحس بالألم حتى بعد إعطاء التخدير.
💉 3. مو معناها نزيد كمية المخدر وخلاص!
إذا فشل التخدير، الأفضل نفكر بـ supplemental anesthesia techniques بدل ما نستمر بإعطاء كميات عشوائية.
مثل:
• PDL injection
• Intraosseous injection
• Intrapulpal injection — إذا وصلنا للـ pulp chamber
📌 Clinical tip:
بـ hot tooth، الـ IANB وحده ممكن ما يكون كافي، لذلك لازم نتأكد من pulpal anesthesia قبل البدء بالعلاج.
🦷✨ الخلاصة:
فشل التخدير بالسن الملتهب مو دائمًا يعني إنك ما سويت الـ injection صح؛ أحيانًا التغيرات اللي يسببها الالتهاب هي اللي تخلي التخدير أصعب. | 74 |
