لأطباء الامتياز الجدد، حياكم في القناة. البعض منكم بدأ في التجميعات من بدري - فحبيت أوجه هذي الرسالة. لا تقلقون، لان نطاق الاختبار محدود جدا و الاسئلة بشكل عام سطحية و اوضح بكثير من الريكولات، و نصيحتي استخدموا مصادر الهيئة الرسمية للتصحيح إذا لاحظتوا اختلاف بين المصادر لان البنك لا يزال قديم.
إلى الان ما حددت إذا ممكن انزل ملف جديد أو لا هذي السنة، لانه يعتمد على الظروف و الوقت، لكن عندي رغبة بتغطية المدسن في المستقبل بنفس طريقة ملفي للسيرجري. | 0 |

A 22-year-old woman, gravida 1 para 0, comes to the office for an initial prenatal visit at 12 weeks gestation. The patient has burning with urination and postcoital bleeding. Pelvic examination shows a friable cervix with a white mucous discharge at the os and in the vagina.
Wet mount preparation shows a pH of 4.1 and >20 white blood cells/hpf. Urinalysis results are as follows:
Specific gravity 1.020
Protein negative
Ketones negative
Blood trace
White blood cells 15-25/hpf
Red blood cells 5-10/hpf
Leukocyte esterase negative

| 17 | Here is a topic that causes some confusion. How to differentiate between several commonly tested STDs in the SMLE concerning abnormal vaginal discharge.
Candida albicans -> thick, cottage cheese odorless vaginal discharge and pruritus. Wet mount shows pseudohyphae and budding yeast. Oral flucoconazole, topical therapy if pregnant. DM2 and recent antibiotic use are a risk factor.
Bacterial vaginosis -> thin, gray fishy vaginal discharge with no other symptoms as it does not cause inflammation. Wet mount shows clue cell. Positive whiff test. >4.5 pH. Metronidazole or clindamycin.
Trichomoniasis -> yellow-green, frothy vaginal discharge, and pruritus. May have inflammatory symptoms such as dysuria and dyspareunia, in addition to vaginal erythema and a friable cervix. Wet mount shows a flagellated motile protozoan. >4.5 pH. Metronidazole or tinidazole; treat sexual partner as well.
Acute cervicitis -> mucopurulent cervical discharge, postcoital spotting, dyspareunia, a friable cervix. Cervical motion tenderness. Wet mount shows >10 WBC/hpf. Empirically treat for two common organisms: Ceftriaxone for gonnorhea, and doxycycline for chlamydia (azithromycin if pregnant). Treat sexual partners.
May progress to tubo-ovarian abscess which are felt as complex adnexal masses on bimanual pelvic exam, and confirmed with an US showing normal blood flow. IV antibiotics.
Side note: US shows an enlarged ovary with decreased or absent blood flow in ovarian torsion, which requires urgent surgical intervention. Ruptured ovarian cysts occur after strenuous activity, with a unilocular mass plus free fluid on US. Treated conservatively if stable. | 0 |
