ثم summation على مستوى bipolar, ganglion cells ثم excitation خلال نصف ساعة
بتكون وصلت لل maximal visual activity

Because it
-composed of cones only
-has no blood vessels
-All retina layers are shedded away of fovea
-there is no convergence
🌟Fovea محاطة ب macula leutea(yellow spot) بتمنع عنها extra UV rays
🌟cones لها pathway ينتهي ب single fiber فال optic nerve
🌟rods بيحصل فيها convergence على مستوى bipolar cells
🌟Optic disc is a blind spot and exists in center of functional visual field
🌟retina is photosensitive to colour and vision وتمتد من Ora serrata لحد optic disc
🌟Rods بتفرق عن ال cones في إنها
- more common
-sensitive to dim light
- see contour not details
-less stimulus intensity
-high convergence وبالتالي
-nerve fibers أقل

3- nerve fiber layer بتعمل angulation وتكون ال optic disc
4- optic disc is a blind spot
🌟فتركيب ال photoreceptors نعرف إن
-outer segment فيها membranous discs فيها photosensitive pigments
-Membranous discs دي separated عن cell membranes فال Rods عشان كده مرتبة القديمة من برا والجديدة جوا
أما فال cones بيبقى ال renewal of segments of diffuse وده عشان يناسب وظيفتها
- neck فيها cilium بينقل البروتين المتكون جديد(membranous discs) لل outer segments
-Inner segment فيها RER دي اللي بتكون ال Membranous discs و mitochondria بتعمل continous pumping of NA outside the inner segment
🌟metarhodopsin ll
هو المسؤول عن generation of photo action potential (hyperporalization)
🌟VIT A بيعمل Buffering of concentration of photoreceptor pigments
🌟all trans retinol هو اللي بيتخزن فال pigment cell layer
🌟Inner segment of Rods بتعمل continous pumping of Na to the outside
So,
RMP= -40
🌟Outer segment فيها Na channels بنقول مجازًا إنها G-linked protein بتعمل leaking of Na inside
🌟Excitaton -----> metarhodopsin II -----> closure of G- protein channels
-----> stoppage of back diffusion of Na
On the other hand:
Na pump تعزف منفردةً----> ⬆️negativity ----> hyperporalization ----> excitation of Rods

🌟White matter = Myleinated Axons+ oligodendrosytes+ few astrocytes +few cell bodies + microbial cells
ال Mylein أغلب تركيبه lipid فبيبقى فاتح
⚠️أحيانًا ال Axons بيتجمعوا ف حزم بنسميها tracts
🌟Grey matter= cell bodies + dendrites+ astrocytes+ + microglial cells
ال cell bodies فيها lipofucin pigments و large nucleus
⚠️Synapes are present in Grey matter

⚠️Infective stage: eggs of Taenia solium
بتكون في لحم الخنازير
☆Mode of infection
- Ingestion of eggs in contaminated food
-External autoinfection ( Hand to mouth)
-Internal autoinfection
لو حصل emesis ال antiperistalic movements بتطلع ال gravid segments المليانة بويضات لل stomach وبت hatch هناك وتطلع hexacains embryo ي pentrate intestinal mucosa ويروح ل SC tissue و Muscles أو لل brain
وعشان كده بديله Antiemetic قبل ما أبدأ علاج
-وجود ال larva بيحفز cellular reaction بتهاجمها فيه كل الخلايا المناعية من أول ال PMNs لحد ال giant cells بعدين بتموت ويحصل لها calcification
وغالبا بتعمل mild infection وتبقى asymptomatic
⚠️Most serious form is neuro- Cysticercosis
بتعمل
-high grade fever
-status epilipeticus
_ convulsions بسبب ال distrusted showers of eggs
-⬆️ICT
-behaviodal disorders
-aphasia
-ocular Cysticercosis
بتصيب كل ال eye structures وتعمل Blurred vision أو loss of vision , iritis,
uveitis, palberal conjunctivitis
-بتعمل eosinophilia لو كانت فال SC tissue
أما لو فال brain بتعمل mild to moderate eosinophilia
☆TTT
Praziquanrel 50mg/ kg daily for 14 days
أما فالشيستوزوما بتبقي single dose, 40 mg

Larval stage of echinococcus granlossus
طالما بتصاب بالlarva يبقى الطور المعدي هو ال eggs
☆MOF:
Ingestion of contaminated food or drink wz eggs
-الأعراض بتعتمد على حجم ومكان ال cyst
بتعدي أكتر حاجة ال liver ثم ال lung ثم ال kidney وال brain
-حجم ال cyst فال liver 20 cm وفال brain 5 cm وقد تصل ل 10cm
🌟pathologic effect is pressure atrophy
فال brain بتعمل ⬆️ICT, Headache, convulsions و epilepsy لأنها permanent cyst موجودة
🌟Diagnosis mainly by CT,MRI

عادةً بتبقى imbedde فال intestinal mucosa (SI)
It's eggs travel with mesenteric capillaries
Commonly to
-myocardium-----> myocarditis
-brain----> ⬆️ICT ----> Headache + repeated convulsions ----> epilepsy
بدي للعيان Anti-inflammatory drugs عشان تخفف عنه بس
🌟Ectopic lesion of schistosoma spp
(Eggs)
- أكتر واحدة بتعمل cerebral هي ال S.Japonicum عشان لها أصغر egg (110 micron ) وبتطلع عدد كبير من البويضات
-بتروح لل CNS وتعمل نفس أعراض ال heterophyes
- أكتر واحدة بتعمل spinal هيS.mansoni
-بتروح لل spinal cord وتعمل فيه inflammation ----> transverse myleitis وتعمل شلل نصفي في صورة LMNL
وتسبب hypotonia, Areflexia , urine retention , hyposthesia

🌟Sparganosis
Diplyllobothrium mansoni and Diphyllobothrium proliferum
هنا الإنسان يعتبر PARATENIC HOST
يعني بيتعدي Accidently وال parasite لا ينمو بداخله
أمال مين ال Definitive host؟
- ال dogs, cats, carnivorous animals
وممكن الإنسان يتعدي بكذا طريقة وكذا stage
1- يشرب مياه فيها Cyclopes اللي هو 1st intermediate host ويحتوي على ال PROCERCOID LARVA
2- ياكل طيور أو ضفادع أو تعابين نيئة اللي هي 2nd intermediate host وتحتوي على PLEROCERCOID LARVA = SPARGANUM
3-زمان كانوا بيضعوا الflesh بتاع الثعابين والصفادع على الأماكن الملتهبة عشان تخفف الالتهاب
ال flesh ده برضو بيكون فيه plerocercoid larva
وطريقة العدوى دي هي السبب المباشر ل ocular infection لما بيضعوها على العين
⚠️ الجزء اللي فات ده موضع أسئلة
-وجود ال sparganum فالجسم بيخليه يعمل حواليها cystic layer ----> 2ry calcification حتى بعد ما تموت

About the use of drugs:
1) Anti epileptic drug used in ventricular dysrhythmia = phenytoin ( Diphenylhydantoin )

3) Drug used for prophylaxis of migraine= valproic acid
4)Drug used for partial seizure , for primary generalized tonic - clonic and for myoclonic seizure = levetiracetam
5)It' use is limited due to sedative effect on children and used as an alternative when therapy with carbamazepine or phynotoin is un successful = phenobarbital
6) Drug has broad spectrum anticonvulsant activity = phenobarbital
7)Treatment of trigeminal neuralgia if carbamazepine is ineffective = phenytoin
8) First choice in treatment of trigeminal and glossopharyngeal neuralgia = carbamazepine
9) First choice for generalized tonic - clonic and partial seizures = carbamazepine
10) First choice in absence seizure ( petit mal ) = Ethosuximide
