✈️Complicated UTI (cUTI) :
Infection in patients with risk factors for infection, treatment failure, or serious outcomes, including:
Male sex
Pregnancy
Postmenopause
Children with features of atypical pediatric UTI
Significant anatomical or functional abnormalities
Immunosuppression
Renal failure
Metabolic disorders (e.g., diabetes)
Infection associated with recent instrumentation or medical devices, e.g.:
Cystoscopy
Indwelling catheters
Drainage devices (e.g., ureteral stents, nephrostomy tubes)
Healthcare-associated UTIs
✈️Uncomplicated UTI
Infection in nonpregnant, premenopausal women without further risk factors for infection, treatment failure, or serious outcomes
✈️Indications for surgery consult in IE include:
Prosthetic valve IE
Valve dysfunction causing heart failure
Uncontrolled infection (e.g., enlarging vegetation, persistent bacteremia)
Perivalvular extension or complications (e.g., abscess, pseudoaneurysm, fistula, heart block)
IE due to difficult to treat organisms (e.g., fungi, MDRO)
High embolic risk (e.g., mobile vegetation ≥ 10 mm, recurrent embolism)
✈️Surgery is required in 50–60% of patients with IE.
✈️Antibiotic therapy is contraindicated in EHEC because it does not improve the disease course and may increase the risk of developing HUS.
✈️Thrombocytopenia, hemolytic anemia, and acute kidney injury are the typical triad for HUS.
✈️Do not use antibiotics if EHEC is suspected. Obtain diagnostic studies and monitor for signs of HUS.
Enterohemorrhagic Escherichia coli (EHEC)
✈️Infants born to HBsAg-positive individuals should receive 1 dose of HBIG and the first dose of HBV vaccine within 12 hours of birth.
✈️HBV infection is not an indication for cesarean delivery.
✈️Hepatitis B vaccination is recommended for all pregnant individuals who have not been previously vaccinated.
✈️HCV infection alone is not an indication for cesarean delivery.
✈️Most acute hepatitis B infections in adults are self-limited. Antiviral therapy is not routinely indicated.
✈️Cranial nerve palsies are the most common early feature of botulism; suspect an alternative diagnosis in patients with no or late onset of cranial nerve palsies.
