⭕️A Significant Role of Vitamin D in Diabetes Prevention: The American Endocrine Society's latest 2024 guidelines recommend vitamin D therapy for individuals with prediabetes to prevent progression to type 2 diabetes. ⭕️Suggested Dosages: ✅ Adults (18-70) : 600 IU/day ✅Adults (over 70): 800 IU/day ⭕️ Prediabetes Diagnosis (non-pregnant) according to the 2025 American Diabetes Association: 💠Hemoglobin A1c: 5.7% to 6.4% 💠Fasting plasma glucose: 100-125 mg/dL 💠2-hour plasma glucose during an oral glucose tolerance test: 140-199 mg/dL

✳️Optimizing Metformin Therapy: “Managing Gastrointestinal Side Effects" Metformin, a cornerstone in type 2 diabetes management, is often associated with gastrointestinal side effects such as nausea, vomiting, diarrhea, and bloating. While these symptoms can significantly impact a patient's quality of life (QOL) and adherence to therapy, several strategies can help alleviate these discomforts. ✳️Understanding the Mechanism: Metformin exerts its effects primarily by reducing hepatic glucose production and increasing insulin sensitivity. However, it can also influence gastrointestinal motility and microbiota, contributing to the observed side effects. ✳️Mechanisms underlying these effects include: ⭕️Increased local serotonin and histamine levels ⭕️Decreased bile salt reabsorption ⭕️Enhanced lactic acid production in intestinal cells ⭕️Alterations in gut microbiota ✳️Strategies for Managing Side Effects: To improve tolerability and adherence to metformin therapy, clinicians can implement the following strategies: 💠Gradual Dose Titration: Starting with a low dose and gradually increasing it over several weeks allows for better adaptation and minimizes side effects. 💠Extended-Release Formulations: Extended-release metformin is often associated with fewer gastrointestinal complaints compared to immediate-release formulations. 💠Probiotics: Supplementation with probiotics may help restore gut microbiota balance and reduce symptoms. 💠Drug Interactions: Concomitant use of certain medications, such as proton pump inhibitors, verapamil, and certain antidepressants, can potentiate metformin's gastrointestinal side effects. Careful medication review is essential. 💠Timing of Administration: Taking metformin with meals can help mitigate symptoms. 💠Dietary Modifications: Reducing sugar intake can minimize lactic acid production in the intestines. 💠Combination Therapy: Combining metformin with other glucose-lowering agents, such as gliptins, may improve tolerability. 💠H. pylori Eradication: Treating Helicobacter pylori infection may alleviate metformin-induced gastrointestinal symptoms.
