اكو فد معلومة حلوة عن الـ Entresto ضروري احنا كصيادلة نُعرُفها. اوّل شيء خلِّي اعرّفكُم على الـ Sacubitril حتّى تتوّضح الصّورة بشكل كامل بعدين. هُوَّ شنو الـ Sacubitril ؟ عبارة عن Neprilysin inhibitors والـ Neprilysin شنو اصلًا ؟ هذا enzyme يسوِّي degradation لعدَّة peptides من ضمنها natriuretic peptides, bradykinin and adrenomedullin. يعني شنو ؟ يعني من راح نجي نُضرُب هذا الـ enzyme راح تكون النّتيجة increased the availability of these peptides which exert favorable effects in heart failure. مثل vasodilatation and natriuretic actions. معلومتي الحلوة وين ؟ اواشك لا تستعجل احنا اصلًا ليش خلّينا هذا الـ Combination معَ بعض ؟ ليش مثلًا مو واحد من مجموعة الـ ACE ؟ لأن أنتَ عندك الـ neprilysin راح يسوِّي degradation للـ angiotension II فضروري جِدًّا نخلِّي معَ علاج يسوِّي blocks rennin-angiotension system. ليش ؟ لأن الـ ACE and neprilysin اثنينهُم راح يسوّون breakdown للـ bradikinin يعني شنو ؟ يعني هذا راح يؤدّي إلى significant increase in the risk of angioedema. لهذا السّبب احنا خلِّينا معَ الـ ARB.

Mouthwash preparation for oral mucositis ( Mary’s Magic Mouthwash ) 🟣 Ingrediants 🟣 💊 nystatin suspension 60 ml of 100,000 I.U/ml 💊Tetracycline 1.5g 💊hydrocortisone 60 mg 💊diphenhydramine HCL syrup enough to bring the total volume up to 240 ml 🟣 Matetials needed 🟣 ⚗️Graduated cylinder(250 mL capacity) ⚗️Mortar and pestle ⚗️Glass container with a lid ⚗️Stirring rod or spoon ⚗️Funnel (optional) 🟣 Preparation methods 🟣 1️⃣- Measure and weight each of the ingrediants 2️⃣- Pour the 60 mL of nystatin suspension into the graduated cylinder 3️⃣- Use the mortar and pestle to crush the tetracycline and hydrocortisone powders together, ensuring they are well mixed and have a fine consistency. 4️⃣- Slowly add the crushed tetracycline and hydrocortisone mixture to the nystatin suspension, stirring continuously to ensure proper mixing. 5️⃣- Slowly add the diphenhydramine HCL syrup to the mixture while stirring, until the total volume reaches 240 mL. Be sure to mix thoroughly to ensure a homogeneous solution. 6️⃣- Carefully pour the prepared mouthwash into the glass container using a funnel,Secure the lid tightly to prevent contamination or evaporation. 7️⃣- Label the container with the name of the mouthwash, its ingredients, and the date of preparation. 🟣 Storage 🟣 Store the mouthwash in a cool, dark place, away from direct sunlight or heat sources. 🟣 Usage instructions 🟣 💁‍♂️Shake the container well before each use. 💁‍♂️Gargle and swish the mouthwash around in your mouth for about 30 seconds, then spit it out.
