🏮CEREBELLUM
• أي أوامر تنزل من الدماغ بتمر على basal ganglia و cerebellum، عشان يتم تنظيمهما بالشكل المطلوب، وجعلها حركة مفهومة ذات مغزى.
• المخيخ مثل الدماغ من حيث الطبقة الخارجية والداخلية منه.
- ويشبه basal ganglia من حيث تواجد nucleus داخله ومن حيث الوظيفة التنظيمية.
- ومثل الدماغ في
✓ Glutamate & aspartate » تحفيز
✓ GABA » تثبيط
🎐 Histological anatomy:
• عنده ثلاث طبقات
- Granular layer
- Purkinje cell layer
- Molecular layer
🎐 Physiological Anatomy:
• عنده أربع طبقات، بإضافة طبقة
- White matter
▪️Cerebral nucleus
- أكبرهم » dentate.
▪️ Projection fibers
• Inferior peduncle
- له علاقة بالاتزان والسمع.
- يكون إلى Medulla oblongata.
• Middle peduncle
- يكون إلى Pons.
• Superior peduncle
- له علاقة بالعين.
- يكون إلى Midbrain.
🎐 Cerebellum Systems
• Vestibulocerebellum
- خاص بالسمع والاتزان.
- يجمع جميع معلومات وضع الجسم من العضلات والمفاصل ويحللهم ويعطي الحركة المطلوبة.
• Spinocerebellum
- يعتبر أكبر جزء.
- يعمل على ضبط المسافات، سواء بالنسبة للنظر أو للسمع.
- فائدة الجمهور في الملعب 🌞 تشتيت تركيز الخصم.
- في الدماغ الجسم يُمثل بالمقلوب حيث الأقدام في الأعلى والرأس في الإسفل، أما بالنسبة للمخيخ العكس فالجسم يمثل كما هو الرأس في الأعلى..
• Corticocerebellum
- يقوم بالحركات المعقدة جداً.
- أي حركة يقوم بها الجسم تخزن في المخيخ.
• المريض المصاب في المخيخ، لا يستطيع عمل حركة finger to nose، وهي من أهم الحركات التشخيصية.
• المخ يعتبر الكمبيوتر
- و thalamus, hypothalamus حق sensory processing.
- و cerebellum, basal ganglia حق motor.
فمثلاً عند الكتابة، نحتاج الإحساس لمعرفة المسافة المناسبة بين الحروف والكلمات، ونحتاج العضلات والمفاصل لعملية الحركة.
🎐 Disturbances:
• Atonia
- لا يستطيع عمل أي برمجة للحركات.
▪️من الأعضاء التي تؤثر عليها الكحول: الكبد والمخيخ.
• Dysmetria
- فيها لا يستطيع المريض عمل حركة finger to nose.
• Rebound phenomenon
- ممكن يؤذي المريض نفسه، لأنه لا يستطيع إيقاف حركته بالوقت المناسب.
• Adiadochokinesis
- لا يستطيع عمل complex movement.
🏮BASAL GANGLIA
• Pyramidal
- وظيفته: الحركة.
• Extrapyramidal
- وظيفته: coordinating، مثله مثل المخيخ.
• Afferent
- يرتبطوا بين بعضهم، ويستقبلوا من الدماغ.
• Efferent
- ترتبط بعدة أماكن منها thalamus, red nucleus, reticular formation.
• Parkinson disease
- لمن يجي يعمل أي حركة بتلاحظ عليه tremor، لأن muscle tone شغالة وما حصلها أي تثبيط من الدماغ.
▪️Red nucleus + reticular formation= related to the arousal mechanism
🎐 Disorders:
• Parkinson disease
- أكثرهم ألماً.
- Unknown cause is the most common.
• Wilson disease
من أجمل علاماته 🌞، أن المريض يكون بعيون زرقاء.
• Kernicterus:
- Physiological jaundice
✓ يكون في أول أسبوعين، ويحدث بسبب تكسر الدم وتغير الهيموجلوبين F.
