بالنسبة الي هذا ثاني مرة Case of repeated vomiting only و يطلع Closed angle glaucoma وحدة من الكيسات الي صعب تخطر ع البال بظل اللود مال البري نايت ♥️♥️

50 yo female presented with pure motor deficit الهستري كالاتي: ايدها اليمنى بلشت ما تتحرك لفترة راجعت عليها .. تم تشخيصها ب Carpal tunnel syndrome بعدها بفترة بدت الحالة تتفاقم ايدها الثانية همه وكفت ما تتحرك اما ك Sensory, position and vibration are intact بعدها صار برجليها وصارت ما تتحرك ابدا اما ك examination لاحظت 👉 Upper limbs = Lower motor neuron lesion Like hyporeflexia, fasciculation, atrophy and flaccidity While 👉 Lower limbs = Upper motor neurone lesion Like hyperreflexia, spasticity.. etc بعدها البيشنت صار عندها Slurred speech and dysphagia ولسانها صار بيه atrophy بس well recognised واخير صار عندها SOB برغم Chest CT normal وما عندها اي امراض مزمنة كل هذا سيناريو (بساطة) تخليه ضمن Motor neurone disease

من شغلات نلاحظها بالطوارىء وبكثرة من يجيك بيشنت عنده typical chest pain وتسويله تخطيط ويطلع عنده ST deviation (elevation or depression) وتنطيه الرجيم Antiplatelets + Anticoagulant بعدها من تخليه ع serial ECG راح تلاحظ صار resolution of ECG changes والم بصدره راح .. هنا لا تنخدع وتخلي البيشنت يطلع! لان هيج كيسات تدخل ضمن موضوع MINOCA (MI with non obstructive coronary arteries) هذا يبقى كل فترة راجعلك للطوارئ with typical chest pain اما اسبابها - Coronary spasm - Plaque disruption, - Spontaneous coronary thrombosis/emboli - Myocarditis .. etc وهاي بطبيعتها response to medical therapy لازم يبقى Observation ع اقل 24 ساعة وتعرف underlying cause اذا ماكو لازم قسطرة تشخيصية واذا طلع ما عنده شي لازم يبقى ع Dual antiplatelet treatment for 12 months and statins والا يبقى اسبوع جايك للطوارئ🥲

📝 Cardiac Asthma من المواضيع جاي يصير بيها misdiagnosis بالطوارئ وكلش كمن .. من يجي بيشنت وعابر 40 سنة وعنده قسطره او ذبحه صدريه او جان يراجع دكتور قلبية الخ.. ويجيك يكح ومختنك وعنده wheezy chest وتسمع وياها basal crepts وتركز وياه بالهستري راح تلكه عنده watery sputum يكلك لونها ابيض او جنها ماي والحاله بليل تزداد عندي اكثر شي من اريد انام ورجليه مورمات هنا تفكيرك اتجاه Cardiac asthma راح تكول شنو ميكانزم وصار اعراضه تشبه ابو asthma 🧐 Secondary to left side HF »» Inability to pump blood out of left ventricle »» Diffuse extravascular accumulation of fluid in the pulmonary tissues »» Increased pressure in the bronchial arteries »» Airway compression and bronchospasm لهذا تريتمنت نفس pulmonary edema O2, Semi-sitting position, GTN and lasix وكل شي يعتمد ع hemodynamic of pt لا تعاملة معاملة asthma وتنطيه hydrocortisone هو غركان وانت تغركه بالزايد 🙂 #emergency #cardio
