📍 Origin

★Arises from the cervical plexus: C3 – C4 – C5 ★Main contribution: C4

Type

Mixed nerve, but functionally mainly: 🟢 Motor: Diaphragm 🔵 Sensory: 🔹Pericardium 🔹Parietal pleura (mediastinal + diaphragmatic parts)

Course in the Neck

Enters thorax: ★Behind the beginning of the brachiocephalic vein ★In front of the subclavian artery ★Separated from vagus nerve by: Thyrocervical trunk ★Descends anterior to: ✰Apex of lung ✰Cervical pleura ✰Suprapleural membrane

🫁 Course in the Thorax

★Accompanies: Pericardiophrenic vessels ★Passes through: Superior mediastinum → Middle mediastinum ➡️ Lateral Relations Related to: ✰Pleura ✰Lung (on both sides)

🔴 Right vs Left Phrenic Nerve

🟠 Right Phrenic Nerve 🔹Runs on the venous side of mediastinum: ✰Right brachiocephalic vein ✰Superior vena cava ✰Right atrium 🔸Continues with: Inferior vena cava 🔸Passes through: Vena caval opening of the diaphragm 🔵 Left Phrenic Nerve 🔹Runs on the arterial side of mediastinum: 🔹Groove between: ✰Left common carotid artery ✰Left subclavian artery 🔹Crosses vagus nerve (posterior → anterior) above aortic arch 🔹Related to: ✰Aortic arch ✰Pericardium covering left ventricle ✰Pierces diaphragm through a separate opening

Important Exam Note

Right phrenic nerve is shorter than left because: 1. Right hemidiaphragm is higher (due to liver) 2. Right phrenic runs more vertically 3. Left phrenic curves around the heart
