Clinical case :مريض عمره 45 سنة، جاي الطوارئ بسبب: ألم مفاجئ شديد في البطن بدأ من ساعتين الألم منتشر في كل البطن (diffuse abdominal pain) البطن مؤلمة جدًا مع أقل لمس (rigidity + guarding) حرارة 38.5 قيء متكرر مفيش خروج براز أو غازات من يوم عنده history لـ NSAIDs استخدام متكرر بسبب آلام المفاصل ما هو التشخيص الأقرب؟ وما أهم الـ differential diagnosis؟

Clinical case :ست عندها 50 سنة، جاية الطوارئ بتشتكي من: ألم شديد في الجزء اليمين من البطن فوق (right upper quadrant pain) الألم بدأ فجأة بعد وجبة دسمة بيروح للكتف اليمين (radiating to right shoulder) معاه غثيان وقيء كمان لاحظت: حرارة خفيفة اصفرار بسيط في العين (mild jaundice)

راجل 40 سنة عنده:
recurrent peptic ulcers مش بتتحسن بالعلاج

Gastrin عالي جدًا

ايه التشخيص ، وكومنت عالاشعة

راجل عنده 58 سنة، بيدخن كتير، بقاله حوالي 3 شهور بيخس من غير سبب واضح. كمان بيقول إنه بيشبع بسرعة جدًا أول ما يبدأ ياكل (early satiety)، ومعاه وجع خفيف في أعلى بطنه.
لاحظ كذا مرة إن لون البراز غامق/أسود. مفيش ترجيع دم.
On examination:
Pale
Left supraclavicular lymphadenopathy (Virchow’s node)
Mild epigastric tenderness
التحاليل:
Hb = 9 g/dL
Microcytic anemia
Stool occult blood: positive
