📚 🩺 متوسط ضغط الشرايين (MAP) - دليل سريري سريع 🔎 ما هو متوسط الضغط الشرياني (MAP)؟ متوسط الضغط الشرياني هو متوسط الضغط في الشرايين خلال دورة قلبية واحدة. إنه يعكس انتشال الأعضاء وهو أحد المعلمين المهمين في المرضى الذين يعانون من الأمراض الحرجة. 🧮 صيغة الخريطة الخريطة = {SBP + 2(DBP)\n ÷(3) أين: • SBP = ضغط الدم الانقباضي • DBP = ضغط الدم الانبطاطي 👉 Diastole يحسب مرتين لأن القلب يقضي المزيد من الوقت في الانفجار. 📊 مثال إذا كان لدى المريض: • ضغط الدم = 120/80 مم زئبق الخريطة = (120 + 2×80) ÷ 3 الخريطة = (120 + 160) ÷ 3 الخريطة = 93 مم زئبق ⚠ الأهمية السريرية • خريطة عادية: ~70-100 مم زهرغ • خريطة ≥ 65 مم زئبق → إنضواء كافٍ للأعضاء • الخريطة < 65 مم زئبق → خطر نقص دمج الأعضاء 🏥 الاستخدامات السريرية الشائعة • إدارة الصدمات الصرفية (خريطة الهدف ≥65) • ICU hemodynamic monitoring • تقييم مدى كفاية الإنعاش • توجيه العلاج بالفاسوبريسور 🧠 اللؤلؤة السريرية قد يكون لدى المريض ضغط الدم الطبيعي ولكن منخفض الخريطة، مما يعني أن الأعضاء قد لا تزال غير مستخدمة. تحقق دائما من MAP في المرضى المصابين بأمراض خطيرة.

DEEP VEIN THROMBOSIS (DVT) — Clinical Features 🔹 Core limb symptoms • Unilateral leg swelling (calf or thigh most common) • Pain or tenderness (may feel deep, throbbing, cramping) • Heaviness/tightness in the affected leg 🔹 Local inflammatory signs • Warmth over the area • Redness or discoloration • Skin may look tense/shiny • Superficial veins may look more prominent 🔹 Calf findings • Localized calf tenderness • Pain may worsen with walking/standing • Sometimes mild swelling only (symptoms can be subtle) 🔹 Common location clue • Clots usually form in deep veins of the leg • Can involve calf (distal), femoral, iliac/pelvic veins • Proximal DVT (femoral/iliac) has higher PE risk 🔹 Asymptomatic possibility • Some DVTs cause few or no symptoms • Clinical suspicion matters—don’t rely on symptoms alone 🔹 Pulmonary embolism (PE) warning • Sudden shortness of breath • Chest pain (pleuritic or pressure-like) • Cough / coughing blood, rapid heart rate, fainting • PE can be life-threatening → urgent evaluation 🔹 High-yield concept (Virchow triad) • Venous stasis (immobility, long travel, hospitalization) • Endothelial injury (surgery/trauma, catheters) • Hypercoagulability (cancer, pregnancy/postpartum, OCPs, inherited thrombophilia) 🔹 Diagnostic approach (high-yield) • Start with clinical probability (Wells score) • Compression/duplex ultrasound = key test • D-dimer helps rule out DVT in low-risk patients (not specific) 🔹 Important differential clue • DVT can mimic cellulitis • Cellulitis often has more diffuse redness, warmth, fever, and skin infection features • DVT = deep venous clot → confirm with imaging 🔹 When to seek urgent care • New one-leg swelling + pain (especially after travel/surgery) • Sudden breathlessness, chest pain, fainting, or coughing blood • Rapidly worsening swelling, severe pain, or high fever Medical disclaimer: This is for education only and not a diagnosis.
