Water Balance Water balance refers to maintaining equal input and output of water in the body. It ensures that cells remain hydrated and function properly. A disrupted water balance can lead to dehydration or overhydration. Water Gain (Sources of Water) Drinking fluids is the main source of water. Water is also obtained from food, especially fruits and vegetables. Metabolic water is produced during cellular respiration. Water Loss (Ways Water Leaves the Body) Kidneys excrete water as urine. Sweating removes water through the skin. Lungs release water vapor during breathing. The digestive system loses water through feces. Control of Water Balance The hypothalamus in the brain detects changes in blood concentration. If the body lacks water, it signals thirst to encourage drinking. It also stimulates the pituitary gland to release ADH (antidiuretic hormone). ADH makes the kidneys reabsorb more water and produce less urine.

Tonicity refers to the effective osmolality that influences water movement across membranes. Small molecules like urea and alcohol move easily across cell membranes and are called ineffective osmoles. Larger molecules like sodium and glucose move more slowly and influence water movement, so they are called effective osmoles. Tonicity is classified into three types: isotonic, hypertonic, and hypotonic fluids. Osmosis is the net movement of water through a semi-permeable membrane. Water moves towards the side with a higher solute concentration (less water). For osmosis to occur, solute concentrations must be different on each side of the membrane. The membrane must be semi-permeable to water but not to solutes. Osmotically active solutes are those that cannot pass through the membrane. Osmolality and Osmolarty Osmolality is the concentration of osmotically active substances per kilogram of water. Osmolarity is the number of osmoles per liter of solution. Osmotic pressure in solutions depends on osmolality. In practice, osmolarity is used instead of osmolality due to easier measurement. Reason 1: Measuring the weight (kilogram) of water is difficult. Reason 2: The difference between osmolality and osmolarity is very small (<1%). --- Changes in osmolality of ECF affect the volume of both ECF and ICF. If osmolality of ECF increases, water moves from ICF to ECF. If osmolality of ECF decreases, water moves from ECF to ICF. Water movement continues until osmolality becomes equal in both compartments.
